ObjectiveTo probe into the impact of comprehensive nursing intervention on the physical and psychological status of enterostomy patients in early colorectal cancer.MethodsThis study is a kind of Quasi experimental research.We have collected materials of 80 patients from 4 patients district of Gastrointestinal surgery of one 3A hospital in Henan Province from March to December of 2016,and these patients all had done Colostomy because of colorectal cancer,which were fit the standards.We randomly made the general survey one and two as the matched group,and the general survey three and five were intervention groups.Each group would have 38 patients.The matched group gave patients 4 times of routine colostomy nursing intervention,lasting 30-60 minutes every time.It included that:(1)preoperative health education and preoperative stoma positioning;(2)changing the process of ostomy bag and the use of ostomy care products;(3)Diet instruction and defecation function training;(4)daily life instruction.Based on routine colostomy care of matched group,intervention group carried on the comprehensive nursing intervention.It included:(1)4 times of Ostomy nursing intervention,it would be lasting 30-60 minutes every time.And the first,third and fourth time of intervention were the same as the matched group's.The second nursing intervention was to improve the process of perfecting ostomy bag.(2)4 times of mind care intervention,every time would be lasting 30-60 minutes.Ostomy care intervention would be performed within days before operation or one week after the operation;mental care intervention were performed two or three weeks after the operation.(3)After discharge of patients,we use WeChat or telephone to do the following up,one time one week.In order to solve the problems of the skin around the stoma,provide psychological knowledge to patients,we think that they are good for recovery.Use the general status questionnaire to investigate the general information of patients.Use ostomy skin tool(OST),herth hope index(HHI),the city of hope-quality of life-ostomy questionnaire(COH-QOL-OQ)to evaluate the physical and psychological status of patients,and evaluate the surrounding skin,hope and life quality of the two groups of patients within 1 week after operation and 3 weeks after operation respectively.Data were collected and recorded in the database.SPSS17.0 was used for statistical analysis.The harmonious analysis of the baseline information of ostomy patients was made using chi-square test,nonparametric test,independent sample t test and other methods,and the mean data ± standard deviation((?)±s)were used to represent measurement data.The scores of OST,HHI and COH-QOL-OQ adopted independent sample t test to compare the surrounding skin,hope level and life quality of the two groups of ostomy patients,and the measurement data were expressed with (?)±s.The test level ? = 0.05.Through comparing the changes of the surrounding skin,hope level and life quality of two groups of research objects before and after the comprehensive nursing intervention,it made analysis and discussions,to evaluate the impact of comprehensive nursing intervention on the physical and psychological status of enterostomy patients in early colorectal cancer.Results1.OST main score through t test.Before the intervention,the scores of the surrounding skin of the control group and the intervention group were respectively 7.18 ± 1.23 and 7.32 ± 2.06.The score difference was not statistically significant(P> 0.05)through t test;After the intervention,the scores of the surrounding skin of the control group and the intervention group were respectively 6.32 ± 1.51,5.48 ± 1.67 through t test,the score difference was statistically significant(P <0.05).2.HHI main score through t test.Before the intervention,the scores of the hope level of the control group and the intervention group were respectively 7.18 ± 1.23 and 7.32 ± 2.06 through t test,and the score difference was not statistically significant(P> 0.05);after the intervention,the scores of the hope level of the control group and the intervention group were respectively 6.32 ± 1.51,5.48 ± 1.67 through t test,the score difference was statistically significant(P <0.05).3.COH-QOL-OQ main score through t test.Before the intervention,the scores of the life quality of the control group and the intervention group were respectively 153.63±10.92,149.34±9.82 through t test,and the score difference was not statistically significant(P> 0.05);after the intervention,the scores of the life quality of the control group and the intervention group were respectively 168.46±13.06,176.21±12.13 through t test,the score difference was statistically significant(P <0.05).Conclusion1.comprehensive nursing intervention can improve the surrounding skin of enterostomy patients in early colorectal cancer,increase patients' hope level and life quality.2.comprehensive nursing intervention can improve the physical and psychological status of enterostomy patients in early colorectal cancer.
