Objective: The five-year survival rate of chronic congestive heart failure is similar to that of malignant tumor;the objectives of current drug therapy mainly are improving cardiac remodeling,influencing long-term prognosis,relieving clinical symptoms and guaranteeing short-term survival rates,etc.By means of venous pumping of rh BNP in small dose to patients with chronic congestive heart failure(CHF)in early phase,this research aims to observe parametric variations of plasma NT-pro BNP,clinical symptom relieving and ultrasonic cardiogram,concentration variations of serum creatinine,sodium and ionic,and judge its clinical effect as well as safety.Methods: This research selected 80 patients diagnosed as chronic congestive heart failure accorded with choosing conditions and hospitalized in Anhui Province Hospital during 2015.2-2016.2.The 80 cases were randomly divided into rh BNP treatment group and control group with 38 and 42 cases in each.Rendered general standardized treatment of cardiac failure to both groups;applied small dose of rh BNP vein for 7 days to rh BNP treatment group on the basis of regular cardiac failure treatment while rendering same dosage of placebo to control group.Applied New York Heart Association(NYHA)cardiac functional grading and ultrasonic cardiogram tests to patients in both groups before and after treatment: recorded parameters such as LVEF,SPAP and LVEDD;determined the values of regular biochemistry(serum creatinine,serum kalium and serum sodium)and plasma NT-pro BNP,and 24 h urine volume of all patients during hospitalization.By taking the event that assisted ventilation is required in cardiac death and cardiac failure and experimental drug is intolerant as outcome,analyzed and compared the curative effect and safety of rh BNP in treating CHF.Results:(1)Serum NT-pro BNP levels of patients in both groups before and after treatment lowered:the level decresed from(4937±3009)pg/ml to(2636±1670)pg/ml in rh BNP group and(4603±2640)pg/ml to(3474±1964)pg/ml in control group,among which the decrease of rh BNP treatment group was more obvious and superior to that of control group,so the difference was statistically different(P<0.05).(2)The improvement of cardiac functional grading after treatment in rh BNP treatment group was more obvious(P<0.05).(3)After the treatment of rh BNP group,LVEF(34±6)% increased to(44±6)%(P<0.05);SPAP and LVEDD were improved compared with that before treatment,but were not obvious,so the difference was not statistically different(P > 0.05);in control group,the change of LVEF,LVEDD,SPAP were not statistically different(P>0.05).(4)During the treatment of rh BNP group,its per capita urine volume(2429±824)ml was higher than that of control group(2136±581),so the difference was statistically different(P<0.05).(5)The serum creatinine,kalium and sodium levels of patients in both groups were not significantly different(P>0.05).Conclusion: Based on traditional treatment of cardiac failure,rh BNP can further enhance LVEF,reduce serum NT-pro BNP,increase urine volume and improve patients' NYHA grading;its clinical effect is superior to traditional treatment with better safety and it does not influence patients' renal function or electrolyte concentration.
