ObjectiveIn recent years,Acupotomy Arthroscope is gradually used to the treatment of Osteoarthritis,which can improve the pain,stiffness and the function of joint.But the research about the effectiveness and duration of Acupotomy Arthroscope are lacking.To research the clinical efficacy of acupotomy arthroscope in the treatment to Knee Osteoarthritis,we conducted a prospective study.Therefore,it can improve the accuracy of diagnosis and make reasonable selection of cases,reasonable treatment expectation and treatment plan,so as to improve the clinical therapeutic effect.MethodsThe subjects were selected from 100 patients with knee osteoarthritis who were admitted to the rheumatology department and outpatient department of the second affiliated hospital of Guangzhou Medical University from June,2016 to June,2017.They were collected into a treatment group and a control group with the inclusion criteria,sixty cases in treatment group with acupotomy arthroscope and the others in control group.After the local anesthesia in the treatment group,the treatment group was performed by using acupotomy arthroscope:adhesion release and other operations through the anterior lateral approach of the knee;meanwhile making continuous joint irrigation.The cut was sutured after operation,and “ diprospan 7mg” was injected into the articylar cavity.After the operation,dressing was changed each 2-3d and stitches was taken out 10-14 d later.In the control group,“ diprospan 7mg” was injected into the knee articylar cavity with osteoarthritis.After the treatment,two groups of patients were treated with oral “Celebrex 100 mg bid “for 4 weeks.Before and after treatment in three monthes and half year,the internationally recognized WOMAC(Universtity Western Ontario and McMaster Universities)osteoarthritis Index and the Lequesne index was used to evaluate the joint pain,joint stiffness,and daily functional limitations in the two group.And record the use of NSAIDs after treatment.Results were statistically analyzed by comparison between the self-contrast method and the two groups.Results1.The differences between pre-and post-treatment of treatment groupResults in the first year after treatment,the knee pain score,the joint stiffness score,the score of limitation of daily activity function,the total scores WOMAC index and Lequesne index of the treatment group were declined respectively.The difference in score between before and 1 to 12 months after treatment was statistically significant(P<0.01).The effect of acupotomy arthroscope on knee osteoarthritis about pain,stiffness,limitation of daily activity function and other aspects is remarkable within one year after treatment.2.The differences between pre-and post-treatment of control groupResults in the first year after treatment,the knee pain score,the joint stiffness score,the score of limitation of daily activity function,the total scores WOMAC index and Lequesne index of the control group were declined respectively.The difference in score between before and 1 months after treatment was statistically significant(P < 0.05),other difference is insignificant.It shows the effect of intra-articular glucocorticoids on knee osteoarthritis,the effect lasted within one month after treatment.3.Comparison of efficacy between two groupsThe difference on each score of the two groups before and after treatment were calculated respectively,and the average score difference between before and after treatment was compared between the two groups.The difference of the treatment group were greater than the control group,it was statistically significant(P<0.05).The effect of acupotomy arthroscope on osteoarthritis was better than intra-articular glucocorticoids within 1 year after treatment.4.Postoperative ComplicationsAmong the 60 patients undergoing Acupotomy Arthroscope treatment,only one patient showed a seepage of synovial fluid on the wound(with clear texture and etiological examination),and recovered after pressure dressing.Other patients did not have any postoperative complications such as seepage,bleeding,infection,joint swelling,poor wound healing,and deep venous thrombosis.ConclusionAcupotomy arthroscope for patients with knee osteoarthritis can quickly relieve the pain and stiffness,improve the knee joint function,reduce the frequency of recurrence.It shows better effect than pure drug therapy,is a safe,effective,flexible therapy with the fast recovery,which is worthy of further popularization and application.
