Based On Data Mining And Analysis, Chief Physician Yan Jun’s Experience In The Treatment Of Chronic Cough

Background:Chronic cough had a wide incidence,the incidence was high,and the etiology is complex,involving a variety of respiratory diseases,digestive system diseases.There were many influencing factors of chronic cough,and the pathogenesis of most diseases was still being studied.Some etiological diagnosis methods of chronic cough were limited,in result some chronic cough had many difficulties in treatment and become one of the key objects that medical field pay close attention to.Traditional Chinese medicine has a wide application prospect in the treatment of chronic cough.Yan Jun,the chief physician,had long been engaged in the clinical and researched of traditional Chinese medicine in the prevention and treatment of lung diseases,and had accumulated a wealth of valuable experience in the treatment of chronic cough.It was of great significance to excavate and sort out it through modern computer technology.Objective:Based on the ancient and modern consilia cloud platform,this paper will collate and excavate Yan Jun’s medical records,and discuss it’s experience in treating chronic cough from the aspects of etiology and pathogenesis,syndrome differentiation and treatment,and the law of prescription grouping.in order to enrich the principle-method-recipe-medicines of traditional Chinese medicine for the treatment of chronic cough.Method:Based on retrospective study,the outpatient medical records of Yan Jun from February 2016 to December 2019 were collected,and the ancient and modern medical record cloud platform was used as the main mining statistical tool.Frequency statistics,chi-square test,association rule analysis and cluster analysis algorithms were used to sort out and excavate the symptoms,syndromes,prescriptions and herbs of medical records.Combined with clinical study,tutor’s published literatures and teaching data,this paper analyzed and discussed cough’s therapeutic methods and the relationship of Yan Jun’s chief physician in the treatment of chronic cough syndrome,syndrome prescription,syndrome and medicine,and the compatibility of traditional Chinese medicine,and summarized the mentor’s experience in treatment.Result:In the outpatient clinic,there were more women with chronic cough than men,the age distribution was wide,the prevalence rate of middle-aged and young people was higher than that of the elderly.Cough with phlegm was common in clinic,such as with white phlegm,with thick phlegm and with a little phlegm.Pharyngeal discomfort was common in chronic cough,poor sleep quality and chest tightness also account for an important proportion,There were 10 kinds of tongue quality manifestations,9 kinds of tongue coating manifestations and 6 kinds of pulse manifestations of chronic cough,among which red tongue,thin coating and slippery pulse were the most common.The exogenous wind syndrome was the type of syndrome that appears most frequently.Chronic cough mainly location lies in the lung,followed by the stomach,and also involved spleen,liver,kidney.The main pathological products of demonstration were "phlegm","fire" and"depression",which were mainly caused by lung,stomach and liver;deficiency syndrome come from qi deficiency,yin deficiency and yang deficiency,which were mainly caused by lung,spleen and kidney.The disease’s nature of chronic cough was mainly excess heat.In the recorded medical records,there were 41 classical prescriptions and non-classical prescriptions,of which the prescriptions for relieving cough and resolving phlegm were more common.In medical records,the common prescriptions based on Erchen Tang were Erhe Fang,Sanhe Fang and Sihe Fang.There were 42 commonly used traditional Chinese medicines with a frequency of more than 20%,with the most frequent frequency of Pinellia temata and honey ephedra;the medicinal properties was mainly warm,the medicine tastes was pungent,and the Channel Tropism was mainly lung meridian.,the efficacies were mainly relieving sore throat,promoting eruption,dry dampness and disperse phlegm,relax the bowel,dispersing wind and heat,depressing qi and resolving phlegm,eliminating phlegm,clearing heat and cooling blood,moistening the lung and relieving cough.The tutor was good at using typical prescriptions:chronic cough patients with wind-cold syndrome such as sneezing,nasal congestion,runny nose and post-nasal drip were treated with Xang’er Zi San;cough with phlegm,Erchen decoction;Xiangsu Powder for chest tightness;Xiao Chaihu decoction for dry pharynx,dry mouth,bitter mouth and daytime cough;Wendan decoction for phlegm yellow and expectoration difficultly;Xiaoqinglong decoction for phlegm,phlegm and phlegm,and Xiaoqinglong decoction for thin white phlegm.The coagulation syndrome of phlegm and qi such as pharyngeal foreign body sensation and cough with thick phlegm were treated with Banxia Magnolia decoction.Based on the analysis of association rules,we got the chronic cough combination of commonly used therapeutic herbs:cassia twig and Radix Paeoniae Alba,Baiqian and Peucedanum,Angelica dahurica,Xanthium and Magnolia,fried burdock,fried silkworm and cicada slough,Schizonepeta tenuifolia,peppermint and uncaria.Conclusion:Yan tutor thought the causes of chronic cough were exogenous and internal injury at both ends,in which the deficiency of internal injury was the basis of the disease,and the syndrome of exogenous wind and internal injury evil excess were the criteria of the disease.In clinical practice,the tutor treats chronic cough from wind phlegm and visceral qi rising and falling abnormality.The principle of treatment was "releasing lung and Evacuate wind,reducing qi and resolving phlegm",and dispersing,descending,warming,moistening and clearing were common therapy.Erchen Tang,Sanru Tang and Zhisu Powder were the basic treating recipes.The tutor good use of drug pairs to regulate the functional activities of Qi.The tutor’s unique clinical thinking was of guiding significance to the clinic.
