| Objective:From the perspective of invigorating the kidney and removing dampness to treat male infertility,observe its clinical efficacy,and compare and analyze its effects on semen parameters,sperm DNA fragmentation(DFI),sperm oxidative stress levels and TCM syndrome scores.To explore the mechanism of the method of tonifying the kidney and removing dampness on reducing the level of sperm oxidative stress and the rate of sperm DNA fragmentation(DFI),providing a new idea for the clinical treatment of male infertility in Chinese medicine.Method:All cases in this study were selected from the Andrology Clinic of Changzhou Affiliated Hospital of Nanjing University of Traditional Chinese Medicine from February 2020 to November 2020.Male infertility patients who met the diagnostic criteria and inclusion criteria,and did not meet the exclusion criteria,were 80 cases in total.22-50 years old,80 patients were divided into 2 groups by random number table method.During the treatment,3 patients were dropped from the treatment group,5 patients from the control group,and finally 72 patients(the treatment group totaled 37 cases,the control group totaled 35 Example)Data access to statistics and analysis.The experimental group was given the traditional Chinese medicine plus and minus polyjing decoction,and the control group was given vitamin E soft capsule orally.The patients in the two groups were treated according to the doctor’s instructions for a course of treatment for 3 months.Semen parameters(semen volume,liquefaction time,sperm concentration,total motility(PR+NP),forward motility sperm ratio(PR),normal morphological sperm ratio),sperm oxidative stress level,sperm DNA fragmentation rate(DFI),record and observe relevant data such as TCM syndrome scores and the number of cases of spouse pregnancy after treatment.Finally,SPSS was used to statistically analyze the obtained data to determine the clinical efficacy.By comparing the sperm oxidative stress level and the sperm DNA fragmentation rate between the groups after treatment,the efficacy of traditional Chinese medicine was evaluated and related mechanisms were explored.Results:In terms of curative effect indicators:the experimental group and the control group can increase semen volume and reduce liquefaction time by comparing the values before and after treatment.The curative effect is significant and there is a statistical difference(P<0.05),but there is no statistical difference in the curative effect between the groups after treatment.Difference(P>0.05);both the experimental group and the control group can increase sperm concentration,forward movement sperm ratio(PR),total motility(PR+NP),normal morphological sperm ratio,and reduce sperm DNA fragmentation rate(DFI)and sperm The level of oxidative stress and the improvement of TCM syndrome scores are statistically different(P<0.05),and the experimental group(Chinese medicine group)is better than the control group,and there is a statistically significant difference(P<0.05).Western medical evaluation:37 cases in the experimental group:3 cured,11 markedly effective,18 effective,5 ineffective,and the total effective rate is 86.49%;35 cases in the control group:2 cured,9 markedly effective,15 effective,and ineffective With 9 people,the total effective rate is 74.29%.In terms of efficacy evaluation of traditional Chinese medicine:37 cases in the experimental group:6 were cured,12 were effective,16 were effective,3 were ineffective,and the total effective rate was 91.89%;35 cases in the control group:3 were cured,7 were effective,19 were effective,and 19 were ineffective.6 people,the total effective rate is 82.86%.Conclusion:1.The method of invigorating the kidney and removing dampness is effective in treating male infertility,which can significantly increase the chance of conception of the patient’s spouse.2.The method of invigorating the kidney and removing dampness can significantly increase sperm density,improve the total vitality of sperm and the speed of sperm forward movement,reduce the rate of sperm deformity and the level of sperm oxidative stress,and protect and repair the integrity of sperm DNA.3.The method of tonifying the kidney and removing dampness has a clear effect on improving sperm oxidative stress and DNA damage.The related mechanism may be through regulating the function of the hypothalamus-pituitary-gonad axis,enhancing the hormone level in the male body,and re-regulating the function of spermatogenic cells Improve the quality parameters of male semen;at the same time increase the body’s antioxidant enzyme activity,inhibit the production of reactive oxygen species,enhance the antioxidant capacity,protect sperm from damage caused by oxidative stress,effectively protect the nucleus and mitochondrial DNA,and avoid damage to the cell structure.The integrity of sperm DNA is guaranteed to protect sperm DNA from damage and at the same time it can perform related repair functions,thereby reducing the level of oxidative stress and the rate of sperm DNA fragmentation. |
