Research On The Medication Rule And Experience Summary Of Professor Lin Ping In The Treatment Of Gastric Precancerous Condition Based On Data Mining

Objective:Data mining technology was used to analyze the medication rule and experience summary of Professor Lin Ping in treating patients with gastric precancerous condition.Methods:The medical records of patients with gastric precancerous condition treated by Professor Lin Ping in the Outpatient department of the Second People’s Hospital affiliated to Fujian University of Chinese Medicine and the 900 th Hospital of the Chinese People’s Liberation Army Joint Logistic Support Force from January 2017 to January 2022 were collected and sorted out,and the medical records that met the requirements were selected based on the inclusion criteria and exclusion criteria.WPS Office Excel,SPSS Modeler 18.0 and SPSS Statistics 26.0 were used to conduct data mining and analysis of medical records through frequency Statistics,association rules and cluster analysis.Results:1.Basic Information: A total of 140 cases were collected in this study,including 304 treatments,including 62 male cases and 78 female cases,with a male/female ratio of 1:1.26.The age range was 21-84 years,with an average age of 52.95 years.Patients aged 51-60 years were the most common,accounting for 35.7% of the total number of patients.2.Frequency analysis: There were 75 kinds of symptoms in 140 medical records of initial treatment.And the symptom of upset stomach is the most common,with a total frequency of 87.14%,followed by insomnia,anorexia,sour regurgitation and other symptoms.The colors of the tongue are mainly red and dark red,and the abnormal shape of the tongue is mainly teeth-printed tongue and enlarged tongue.The main coatings on the tongue are yellow and greasy coated tongue and thin yellow coated tongue,and sometimes visible the abnormal clinical manifestations of varicose hypoglossal veins and purplish dark hypoglossal veins.The main pulse conditions are thready pulse and string-like pulse."Spleen deficiency and qi stagnation" was the basic syndrome differentiation in the initial treatment and the last treatment.The frequency of "heat,dampness,blood stasis,phlegm" was more than 10% in the initial treatment and the last treatment,and the syndrome of combined spleen and kidney deficiency and combined with blood stasis appeared more frequently in the last diagnosis.There were 10 kinds of drugs with the frequency of more than 50%,which were Poria Cocos,Tangerine Peel,Bupleurum Chinense DC,Fried Atractylodes Macrocephala,Perilla Seed,Radix Paeoniae Alba,Cortex Albiziae,Rhizoma Corydalis,Trichosanthes Kirilowii Maxim,Lumbricus.The five most frequently used drugs in the two visits were tonify deficiency medicine,qi-regulating drugs,expectorant cough suppressant and anti-asthmatic drug,damp-clearing drug and heat-clearing drugs.3.Association Analysis: Based on association rule algorithm,when support degree≥80% and confidence degree ≥95%,the three drug combinations with the strongest association degree were Bupleurum Chinense DC,Tangerine Peel and Poria Cocos.Based on association rule analysis of syndrome differentiation profile with clinical manifestations and drugs,when support degree ≥4% and confidence degree ≥95%,the combination of clinical manifestations-syndrome differentiation profile and drugs-syndrome differentiation profile with strong correlation degree can be obtained as follows:(1)Patients with symptoms of wakefulness,bitter taste,dry mouth,acid regurgitation,poor appetite,sticky stool,yellow urine,red tongue,yellow and greasy coated tongue and string-like pulse are mostly identified as heat syndrome.And professor Lin often treat them with traditional chinese medicine such as Herba Artemisiae Capillariae,Scutellaria Baicalensis,Coptis Chinensis,Chinese Pulsatilla and Caulis Bambusae In Taeniis;(2)Patients with symptoms of acid regurgitation,poor appetite,sticky stool,yellow and greasy coated tongue and string-like pulse are mostly identified as wetness syndrome.Professor Lin often treat them with traditional chinese medicine such as Herba Artemisiae Capillariae,Fructus Amomi,Caulis Bambusae In Taeniis,Coptis Chinensis,Poria Cocos and Tangerine Peel;(3)Patients with stomach tingling,dark tongue varicose hypoglossal veins,purplish dark hypoglossal veins and unsmooth pulse are mostly identified as blood stasis syndrome.Professor Lin often treat them with medicine such as Lumbricus,Radix Paeoniae Rubra,Trogopterus Dung and Radix Salviae Miltiorrhizae.4.Cluster Detection: Through k-means clustering analysis,four core prescriptions were obtained :(1)Poria Cocos,Tangerine Peel,Bupleurum Chinense DC,Fried Atractylodes Macrocephala,Perilla Seed,Radix Paeoniae Alba,Rhizoma Corydalis,Fructus Tritici Levis,Fructus Amomi,Areca Catechu;(2)Poria Cocos,Tangerine Peel,Bupleurum Chinense DC,Perilla Seed,Cortex Albiziae,Rhizoma Corydalis,Trichosanthes Kirilowii Maxim,Fructus Amomi,Radix Paeoniae Rubra,Raw Atractylodes;(3)Poria Cocos,Tangerine Peel,Bupleurum Chinense DC,Fried Atractylodes Macrocephala,Perilla Seed,Radix Paeoniae Alba,Cortex Albiziae,Lumbricus,Medicated Leaven,Coix Seed,Rhizoma Dioscoreae;(4)Poria Cocos,Tangerine peel,Bupleurum chinense DC,Fried Atractylodes Atractylodes,Perilla Seed,Radix Paeoniae Alba,Rhizoma Corydalis,Caulis Bambusae In Taeniis,Fructus Amomi,Rhizome Pinelliae Preparata,Radish Seed,Herba Artemisiae Capillariae.Conclusion:1.Professor Lin believed that the basic pathogenesis of the gastric precancerous condition was "spleen-deficiency and qi-stagnation",phlegm and blood stasis were the key factor of the progression of the disease,damp-heat was common accompanying symptom in the course of the disease,and in the later stage of the disease,"excessive superficial" gradually disappeared,the symptoms of spleen and kidney deficiency were often accompanied.2.On the basis of the "Tiaozang Yunqi Decoction" Professor Lin added and subtracted some traditional chinese medicine to invigorating spleen,soothing liver and regulating the qi function of the viscera.For patients with gastric precancerous condition,professor Lin often added Lumbricus to achieve the purpose of dissolving stagnant and eliminating phlegm and freeing channels.And according to the different syndromes of heat,dampness,phlegm,blood stasis,cold and food accumulation,the traditional Chinese medicine with the functions of clearing heat,eliminating damp,resolving phlegm,removing blood stasis,dissipating cold and removing food accumulation was added respectively.3.In the late stage of treatment,professor Lin focused on regulating the imbalance of Yin and Yang of spleen and kidney.Professor Lin’s principle in treating gastric precancerous condition is to guard the pathogenesis and comprehensive treating.
