Discussion On Chief Physician Yv Xiumei Clinical Experience And Medication Rules Of Treating Chroni Glomerulonephritis Based On Data Mining

Objective:Through collecting and sorting out the cases of chronic glomerulonephritis patients treated by tutor,this study uses the "Traditional Chinese Medicine Inheritance Assistant Platform(V2.5)" to conduct data mining,sorting and analysis,study the medication rules of the tutor in treating chronic glomerulonephritis,and analyze the clinical diagnosis and treatment ideas of chronic glomerulonephritis based on the clinical experience of the tutor,so as to provide new reference for the traditional Chinese medicine treatment of chronic glomerulonephritis.Methods:This study collected cases of chronic glomerulonephritis treated by tutor from October 2019 to October 2022.The research will record the cases that meet the requirements into the "Chinese medicine inheritance assistance platform(V2.5)",and conduct data mining and analysis through the platform to obtain the results.Results:1.This study included a total of 148 patients and 2084 medical records,including 83 males and 65 females.2.There are 7 patients aged20-29,accounting for 4.73%,26 patients aged 30-39,accounting for 17.57%,41 patients aged 40-49,accounting for 27.70%,47 patients aged 50-59,accounting for 31.76%,23 patients aged 60-69,accounting for 15.54%,and 4 patients aged70-80,accounting for 2.70%.3.A total of 171 symptoms were included in this study,of which the top five symptoms with the highest frequency were foam urine(362,17.37%),lower limb edema(321,15.40%),poor sleep(132,6.33%),fatigue(119,5.71%),and constipation(110,5.28%).4.A total of 30 kinds of tongue images were included in this study.The first three tongue nature were fat tongue(304,14.59%),dentate tongue(111,5.33%)and red tongue(76,3.65%),and the first three tongue coating were thin and white coating(1016,48.75%),thin and yellow coating(261,12.52%)and thick and yellow coating(120,5.76%).5.A total of 23 pulse patterns were included in this study,and the first three were chord pulse(751,36.04%),deep chord pulse(459,22.02%)and deep pulse(409,19.63%).6.The top five syndromes included in this study are damp-heat syndrome(655,31.43%),qi stagnation syndrome(327,15.69%),dampness and turbidity syndrome(275,13.20%),spleen and kidney yang deficiency and dampness and turbidity syndrome(184,8.83%),spleen and kidney qi deficiency and dampness and turbidity syndrome(165,7.92%).7.A total of 275 drugs were included in this study,and the top 10 drugs with the frequency of occurrence were Rosa laevigata(1230),Poria cocos(1072),Glycyrrhiza uralensis(867),Astragalus complanatus(799),Bixie(713),Pericarpium Citri Reticulatae(702),Astragalus membranaceus(617),Salvia miltiorrhiza(580),Asiatic Plantain Seed(572),and Pinellia ternata(550).8.This study included 275 traditional Chinese medicines,with a total frequency of 26145.The top 10 drugs with efficacy were Qi tonifying drugs,Qi regulating drugs,diuretic and swelling reducing drugs,essence tonifying and urine reducing and stopping drugs,diuretic and drenching drugs,digestive drugs,Yang tonifying drugs,blood activating and menstrual regulating drugs,dispersing wind heat drugs,and clearing heat and cooling blood drugs.9.This study involved a total of 275 traditional Chinese medicines,including five types of medicinal properties:cold,hot,warm,cool,and flat.The frequency ranged from high to low,followed by warm drugs(9120,37.41%),flat drugs(7533,30.90%),cold drugs(6581,27.00%),cool drugs(946,3.88%),and hot drugs(196,0.80%).10.The five flavors of the drug in this study involve six types:sour,bitter,sweet,spicy,salty,and astringent.The frequency ranges from high to low: sweet(11878,32.15%),bitter(10695,28.95%),spicy(8813,23.86%),sour(3128,8.47%),astringent(1990,5.39%),and salty(436,1.18%).11.This study involved a total of 12 drug meridians,including liver,heart,spleen,lung,kidney,gallbladder,stomach,pericardium,triple energizer,large intestine,small intestine,and bladder.The frequency from high to low was spleen meridian(11713,17.71%),liver meridian(9076,13.72%),kidney meridian(8968,13.56%),lung meridian(8551,12.93%),stomach meridian(8120,12.28%),heart meridian(6040,9.13%),large intestine meridian(3913,5.92%),bladder meridian(3755,5.68%)Biliary Meridian(2510,3.79%),Small Intestine Meridian(1994,3.01%),Pericardium Meridian(787,1.19%),and San Jiao Meridian(713,1.08%).12.Based on the analysis of new methods,a correlation degree of5 and a penalty degree of 2 were set,and the combination was extracted to obtain a total of 10 core drug combinations,namely fennel＿Yanhusuo＿Melia azedarach,fennel＿Dried ginger＿Myrrh,Zedoary Turmeric＿Scutellaria baicalensis＿Fructus Aurantii,Scutellaria baicalensis＿Tangerine peel＿Pinellia ternata,Scutellaria baicalensis＿Fructus Aurantii＿Orange peel,white peony＿Licorice＿Chaihu＿Mint,Coix Seed＿Atractylodes macrocephala＿Chuan Niu Xi,Pei Lan＿Chuan Niu Xi＿Gypsum,papaya＿Eucommia ulmoides＿Ox knee,Eucommia ulmoides＿Ox knee＿Sang Jisheng.13.Based on the core drug combination,with a correlation degree of 5 and a penalty degree of2,five new formula combinations were obtained,namely fennel＿Yanhusuo＿Sichuan Melia Seed＿Dried ginger＿Myrrh,Zedoary Turmeric＿Scutellaria baicalensis＿Fructus Aurantii＿Tangerine peel＿Pinellia ternata and Scutellaria baicalensis＿Fructus Aurantii＿Tangerine peel＿White peony＿Licorice＿Chaihu＿Mint,Coix Seed＿Atractylodes macrocephala＿Chuan Niu Xi＿Perrin＿Gypsum,papaya＿Eucommia ulmoides＿Ox knee＿Sang Jisheng.Conclusion:1.The mentor pays attention to spleen and kidney deficiency in the treatment of chronic glomerulonephritis,while emphasizing pathogenic factors such as pathogenic excess.2.The mentor clinical treatment focuses on nourishing the spleen and kidney,caring for innate and acquired root,clearing heat and dampness,grooming functional activity of Qi,regulating Qi and blood and water equally,and eliminating external evil and bewaring of repetition.3.Post drug cares value conditioning diet and living subjects,adjustment to body habitus,and peace of mind.
