| Objective To investigate the effects of Zishen Yangjing Decoction on patients with diminished ovarian reserve(DOR)-induced infertility by Deficiency of Essence and Blood from the aspect of clinical research,in order to provide an effective clinical experience in the treatment of DOR induced infertility using traditional Chinese medicine(TCM).Methods Seventy four patients with dor infertility of blood essence deficiency type who were treated in the Department of traditional Chinese medicine of Shandong provincial hospital from October 2021 to December 2022 were included in this study.They were randomly divided into two groups,37 cases in each group.The control group was given estradiol tablets / estradiol dydrogesterone tablets(fenmaitong 1 / 10mg)orally,while the observation group was given Zishen Yangjing decoction combined with estradiol tablets /estradiol dydrogesterone tablets(fenmaitong 1 / 10mg)orally.Both groups were treated for 3 menstrual cycles.The basal sex hormone levels(FSH,LH,E2),anti Mullerian hormone(AMH),antral follicle count(AFC),ovarian volume(MOD),pregnancy rate,TCM syndrome score and safety indicators of the two groups were observed before and after treatment.The ovulation rate and pregnancy of the patients were recorded after 3 menstrual cycles of follow-up.Result 1.65 patients finally completed the study,including 32 cases in the observation group and 33 cases in the control group.2.The total effective rate of the observation group was 96.88%,and that of the control group was 75.76%,P < 0.05,indicating that Zishen Yangjing decoction combined with fenmutong has a more definite effect on dor infertility of blood essence deficiency type.3.The ovulation rate of the observation group was higher than that of the control group during treatment and follow-up(P < 0.05);The number of pregnancies in the observation group was more,but there was no statistical difference in the pregnancy rate between the two groups(P > 0.05).4.Compared with the values of serum sex hormones and AMH before and after treatment,the medication in both groups could decrease the levels of FSH and increase the levels of E2 in infertile patients with dor(P < 0.05),and the observation group was superior to the control group(P < 0.05;Medication in both groups did not significantly alter the LH and AMH indices of the patients(P > 0.05).5.Compared with the ovarian indicators before and after treatment,the observation group was statistically significant in elevating ovarian mod values,AFC numbers,and the control group in increasing ovarian AFC values(P < 0.05);The combination of Zhishen Yangjing Decoction and fenmutone was more effective than fenmutone alone in improving ovarian mod values and increasing the number of AFCS(P < 0.05).6.The TCM syndrome scores of the two groups before and after treatment were statistically significant(P < 0.05),indicating that the two groups of drugs can improve the clinical symptoms of DOR infertility patients with blood essence deficiency in different degrees;After treatment,the scores between the two groups were compared,which showed that the effect of Zishen Yangjing decoction combined with fenmatong was better than that of fenmatong alone in improving waist and knee soreness,dry hair,tinnitus,mental fatigue,insomnia and dreaminess,dry eyes,menstrual period and other aspects(P <0.05).Conclusion 1.Zishen Yangjing decoction combined with fenmotong can effectively improve the related clinical symptoms of DOR infertility patients with blood essence deficiency,such as waist and knee soreness,decreased sexual desire,dry hair,tinnitus,mental fatigue,pale complexion,insomnia,dreaminess,dry eyes,less menstrual volume,menstrual period,etc.2.Zishen Yangjing decoction combined with fenmutong in the treatment of DOR infertility of essence blood deficiency type can increase the clinical pregnancy probability by adjusting hormone levels,improving ovarian function,and increasing ovulation rate. |
