ObjectiveThis topic is a retrospective study of the tutor’s previous clinical diagnosis and treatment of CRF cases.Through the traditional Chinese medicine inheritance calculation system,the characteristics of the tutor’s treatment of CRF are explored and analyzed,and the theoretical connotation of the method of “Yishen Jiangzhuo” is explored.To predict and explore the potential mechanism of core drugs through network pharmacology research;summarize the clinical experience of the tutor in the diagnosis and treatment of CRF.Methods1.From June 2017 to September 2022,232 cases of CRF patients diagnosed and treated in the outpatient and ward of the Second Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine were collected and screened,and the complete medical records that met the inclusion criteria were collected.2.Use the traditional Chinese medicine inheritance computing system to complete the entry of relevant data such as first diagnosis information and prescription drugs;the basic situation of patients,frequency of drug use,drug property,taste,meridian tropism,core drugs,commonly used drug combinations,drug core combinations and other statistical data were obtained.3.Network pharmacology research,through the TCMSP database,screen and obtain the active ingredients and targets of core drugs;the Uniprot database was used to standardize the target protein of the effective components of the drug;disease gene targets were searched in Gene Cards,OMIM,Pharm Gkb and other databases;the intersection targets of core drugs and diseases were obtained by Venny software.The intersection targets were imported into STRING database to construct PPI network.According to PPI related data,Cytoscape software was imported to screen core targets.R software was used to complete GO and KEGG enrichment analysis to explore the mechanism of core drugs in the treatment of CRF.Results1.Basic information:Patient gender and age statistics showed: 232 cases of medical records,139 males;93 females;the age ranged from 19 to 82 years old,including 190 cases over 50 years old.Primary disease statistics showed: 98 cases of diabetic nephropathy;73 cases of chronic glomerular disease;34 cases of hypertensive renal damage;23 cases of uric acid nephropathy;the other 4 cases.Statistics of TCM syndromes showed: 147 cases of spleen and kidney deficiency,damp turbidity and blood stasis syndrome;liver and kidney yin deficiency,blood stasis syndrome 46 cases;qi and Yin deficiency,damp heat syndrome 24 cases;yin and Yang deficiency,wet stasis block syndrome in 11cases;the other 4 cases.2.Analysis of drug characteristics:232 cases of prescriptions,involving 189 flavors of traditional Chinese medicine,the frequency of use of the top ten flavors were: Astragalus membranaceus 200 times,Dioscoreae rhizoma 195 times,Hedyotis diffusa 182 times,Radix salviae miltiorrhizae 177 times,Codonopsis radix 172 times,Radix te rhizoma rehi 168 times,Cortex eucommiae 156 times,Herb of spicate clerodendranthus 145 times,rhizoma smilacis glabrae 140 times,Membrane of chicken gizzard 136 times.The results of drug four gas showed: flat 1600 times,warm 1334 times,cold 1248 times,cool 168 times,hot 11 times.The results of drug five flavors showed: sweet taste 2845 times,bitter taste 1862 times,pungent taste 828 times,sour taste 536 times,salty taste 190 times.Drug meridian results showed that: liver meridian 2300 times,spleen meridian 2052 times,kidney meridian 1842 times,lung meridian 1670 times,stomach meridian 1188 times,etc.Commonly used drug combinations: such as Astragalus membranaceus and Dioscoreae rhizoma,Astragalus membranaceus and Radix te rhizoma rehi,Codonopsis radix and Radix salviae miltiorrhizae;The drug core combination was 4 groups.3.Network pharmacology research results: 120 active ingredients of core drugs were obtained,such as quercetin,kaempferol,luteolin,diosgenin,etc.There were 254 drug targets,1754 disease targets and 133 drug-disease intersection targets.The core targets were mainly AKT1,STAT3,JUN,MAPK3,etc.GO enrichment obtained 2301 biological processes,61 cell compositions,and 162 molecular functions.There were 174 KEGG pathways,and the main signaling pathways were AGE-RAGE,lipid and atherosclerosis,IL-17,HIF-1,etc.Conclusion1.Tutor believes that the key to the pathogenesis of CRF lies in the deficiency of spleen and kidney,damp turbidity and blood stasis,which belongs to the syndrome of deficiency in origin and excess in superficiality,and the combination of deficiency and excess.2.Tutor clinical syndrome differentiation and treatment of CRF,good use of Astragalus membranaceus,Codonopsis radix,Dioscoreae rhizoma,Cortex eucommiae,Membrane of chicken gizzard,flat,warm medicine,kidney and spleen,to consolidate its foundation;supplemented by Radix salviae miltiorrhizae,Radix te rhizoma rehi,Hedyotis diffusa,Herb of spicate clerodendranthus,rhizoma smilacis glabrae and other bitter,cold and cool products,in order to clear the discharge,removing blood stasis and dredging collaterals,detoxification and detoxification;3.In the clinical practice,the tutor is good at using the combination of drugs such as drugs,taking their mutual synergy,reducing toxicity and increasing efficiency;and according to the characteristics of syndromes,medication according to law,with the syndrome.4.The network pharmacology research shows that the core drugs have the characteristics of multi-component,multi-target and multi-channel regulation.Through the synergistic effect of drugs,they play a variety of mechanisms such as anti-oxidative stress,anti-inflammatory response,regulation of apoptosis,regulation of immunity,etc.AND alleviate pathological damage such as glomerulosclerosis and renal tubular interstitial fibrosis,protect renal function,and delay the progression of CRF. |
