Objective:To observe the effect of Taohong Siwu decoction on inflammatory response after intramedullary nail fixation of tibiofibular fractures.Methods:Patients with tibiofibular fractures who met the inclusion criteria and had completed intramedullary nail internal fixation in the Fuzhou Second Hospital from January 2022 to January 2023 were collected in this study.They were divided into the experimental group and the control group by random number table method,with 30 cases in each group.Patients in the two groups were given celecoxib capsule for pain relief and nadroparin calcium injection for thrombosis prevention before operation.All patients were performed under general anesthesia with nerve block anesthesia and tibiofibular fracture closed reduction and intramedullary nail fixation.The control group was treated with celecoxib capsule for pain relief,nadroparin calcium injection for thrombosis prevention,and mannitol injection for swelling.Experimental group: on the basis of the control group,Taohong Siwu decoction was added from the first day to the sixth day after operation.The levels of IL-1β,IL-6,WBC,CRP and swelling of the affected limb on the 1st,3rd,5th and 7th day after operation were statistically analyzed,and the adverse reactions were recorded.Results:1.Comparison of general data: a total of 60 patients who met the inclusion criteria were collected in this study,and no cases were dropped out.There was no significant difference in gender,affected side,waiting days before operation,operation time,age,and circumference of the healthy side between the two groups(P > 0.05).The baseline data of the two groups were comparable.2.IL-1β: there was no significant difference in the concentration of IL-1β between the first day and the third day after operation(P > 0.05),but there was significant difference between the fifth day and the seventh day after operation(P < 0.05).The concentration of IL-1β in the two groups showed a gradual downward trend,and the change rate of the experimental group was faster than that of the control group from the first day to the seventh day after operation.3.IL-6: there was no significant difference in the concentration of IL-6 on the first day after operation(P > 0.05),but there was significant difference in the concentration of IL-6 on the third,fifth and seventh day after operation(P < 0.05).The concentration of IL-6 in the two groups showed a gradual downward trend,and the change rate of the experimental group was faster than that of the control group from the first day to the third day after surgery.4.WBC: there was no significant difference in WBC between the first day and the third day after operation(P > 0.05),but there was significant difference in WBC between the fifth day and the seventh day after operation(P < 0.05).The WBC values of the two groups showed a gradual downward trend,and the change rate of the experimental group was faster than that of the control group from the first day to the seventh day after operation.5.CRP: there was no significant difference in CRP value between the first day and the third day after operation(P > 0.05),but there was significant difference between the fifth day and the seventh day after operation(P < 0.05).The CRP value of the two groups showed a trend of first increase and then decrease,and the decrease rate of the experimental group was faster than that of the control group from the 3rd to the 5th day after operation.6.Swelling value: there was no significant difference between the first day and the third day after operation(P > 0.05),but there was significant difference between the fifth day and the seventh day after operation(P < 0.05).The swelling value of the two groups showed a gradual downward trend,and the change rate of swelling value of the experimental group was faster than that of the control group from the 3rd to the 5th day after surgery.Conclusion:Taohong Siwu decoction can reduce the inflammatory response index after intramedullary nail fixation of tibiofibular fractures,and has certain effects on swelling and anti-inflammation.
