Professor Piao Chunli’s Clinical Experience In Treating Obese Type 2 Diabetes Mellitus

Objective: To collect the cases of obese patients with type 2 diabetes diagnosed and treated by Professor Piao Chunli,and analyze the data after information integration and screening.The supervisor,Professor Piao has a remarkable clinical effect in the treatment of obesity type 2 diabetes.Her clinical experience is worth summarizing.Combining with the clinical follow-up during the study period,the tutor’s academic thoughts combined with clinical practice experience,from theoretical study to practical experience,and then to the combination of learning and thinking Discuss Professor Piao Chunli’s clinical experience in the treatment of obese type 2 diabetes,and provide new ideas for clinical treatment of this disease to improve the curative effect.Methods: The cases mainly came from Shenzhen Hospital of Guangzhou University of Traditional Chinese Medicine(Futian).From October 2020 to December 2022,Professor Piao was diagnosed with obesity type 2 during outpatient visits and ward rounds in the Department of Endocrinology of the Chinese Medicine Center.For diabetic patients,the medical history information is completely recorded,and all medical record information and traditional Chinese medicine prescriptions are signed and confirmed by Professor Piao.The content of the medical record includes the general condition of the patient,clinical information on the four clinics,medication,etc.The medical record data were preprocessed,and entered into excel to establish the original database after standardization and screening,and then the case data that did not meet the inclusion and exclusion criteria were eliminated,and the data analysis was carried out after refinement and screening.Based on the calculation module of the traditional Chinese medicine inheritance calculation platform(3.0),the recorded medical record data is passed through the modules of "statistical analysis","prescription analysis" and "symptom analysis" in the system,aiming at the four flavors,five flavors and attributes of medicines in this study.After the statistical analysis of frequency and frequency;finally,through the association rules and cluster analysis of the system,the statistical results of the tutor’s prescription prescriptions were obtained,and the results of core traditional Chinese medicine combinations and mining new prescriptions for the treatment of this disease were obtained.Results: Among the 200 patients,males accounted for slightly more than females.The most common syndrome of obesity type 2 diabetes was stagnant heat in the liver and stomach,followed by spleen deficiency,stomach heat and deficiency of both qi and yin,and blood stasis and phlegm dampness were often seen.In terms of medication: cold traditional Chinese medicines are used most frequently,followed by warm traditional Chinese medicines;in terms of five flavors,bitter traditional Chinese medicine are the most,followed by sweet traditional Chinese medicines;in terms of prescriptions: set ‘Syndrome of stagnation of liver and stomach heat’,and set ‘Number of clusters’ to 4,a total of 129 prescriptions were obtained,and the prescriptions and syndrome types were set to‘concurrent stasis’,and the ‘number of clusters’ was set to 4.A total of 121 prescriptions were obtained,and the prescriptions and syndrome types were set to ‘concurrent phlegm and dampness’,and ‘number of clusters’ If it is set to 4,a total of 39 prescriptions are obtained,and the K-means clustering algorithm is performed.The ‘number of clusters’ is set to 4,and 4,4,and 3 new prescription combinations are obtained respectively.Conclusion: Among the 200 patients,the proportion of male patients was slightly more than that of female patients;the most common syndromes of obesity type 2 diabetes were liver-stomach stagnation-heat syndrome,spleen-deficiency stomach-heat syndrome and deficiency of both qi and yin,and blood stasis and phlegm-dampness were also seen etc.Regarding the frequency of traditional Chinese medicine use,the frequency of use of cold traditional Chinese medicines is the highest,followed by that of warm traditional Chinese medicines;the results of the analysis of the four characteristics and five flavors of traditional Chinese medicines show that bitter traditional Chinese medicines are the most used in this study,followed by sweet traditional Chinese medicines;association rules and prescription analysis,set ‘liver-stomach stagnation-heat syndrome’,‘concurrent blood stasis’ and ‘concurrent phlegm-dampness’ to obtain different correlation results;finally,K-means clustering algorithm was used to obtain the clinical prescriptions commonly used by tutors in treating obesity type 2 diabetes mellitus And medicine pairs: Jiedu Tongluo Tiaogan Fang,Dried Ginger,Huangqin,Huanglian and Ginseng Decoction,Rhubarb and Huanglian Xiexin Decoction;Banxia Xiexin Decoction;weight loss medicine against cassia seed and lotus leaf;diuresis and swelling medicine against astragalus and Alisma.
