The Clinical Study On Therapeutic Effects Of "GuBen JiuXin Soup" In Treating Patient With Chronic Heart Failure

Objective1.To observe and evaluate the therapeutic effects and the function machanism of "GuBen JiuXin Soup" in treating chronic heart failure (CHF) inpatients with syndrome of Qi deficiency of the heart and Yang deficiency of the kidney and stagnation of blood and phlegm;To study the relationship of CHF syndromes with both changes of cardiac fractions and BNP and using Doppler ultrasound color cardiogram to test cardiac functions.2. To investigate the change of B-type natriuretic peptide (BNP) and cardiac functions and testing of cardiac structural change by using Doppler ultrasound color cardiogram in patients with CHF. To discuss the relationship between syndrome differentiation of TCM and the change of BNP and testing of cardiac structural change in patients with HF. The difference between the syndrome differentiation of TCM and the change of BNP and cardiac structural change can provid the experimental diagnostic basis for the study of zheng diffetentiation-classification in CHF. Methods1. In this study, 60 patients with CHF (New York Heart Association class Ⅲ~ Ⅳ) Were randomly assigned to the treating group of receiving "GuBen JiuXin Soup" (30 patients belong to syndrome of Qi deficiency of the heart and Yang deficiency of the kidney and stagnation of blood and phlegm), and the control group (30 patients) On a basic medical therapy including digitalis , diuretics , ACEI and β -blocker and (or) Expander drug for 1~2 weeks. We investigate the clinical efficacy of the treating group and the control group in this study and the BNP change. 2.168 and 100 cases of CHF were observed to study the objectification ofsyndrome differentiation in TCM, using Doppler ultrasound color cardiogram to test LVEF and FS and IVST and LVPWT and LVDd or checking BNP. Results1. the treating group of receiving " GuBen JiuXin Soup" could remedy significantly heart failure and improve evidently clinical symptom and sign and cardiac function, the treating group did better than control group (P<0. 05, P<0. 01);As for evaluating the total treating effect by TCM Standard, the treating group did better than Constasting group (P<0.05);In decreaseing the level of BNP, the treating group did better than Constasting group (P<0. 01)2. In providing the experimental diagnostic basis for the study of zheng diffetentiation-classification in CHF, It showed the significant differences between Yang Deficiency of both Heart and Kidney did better and Qi deficiency in testing of LVEF and FS and IVST and LVPWT and LVDd and BNP (P<0.01, P<0. 001) . "GuBen JiuXin Soup" is safe and efficacious for therapy of chronic heart failure.4. This investigation is of helpful study to treatment of CHF using TCM, and provid fine foundation for further study of this field in future.
