Research On The Migrant Workers' Medical Insurance System

With the further development of reform and open, under the background of the rapid rise of agriculture labor rate and the construction of urbanization and industrialization, a large number of rural residual labor force come off the countries into cities to engage in non-agricultural industry and become migrant workers. After the development for nearly 30 years, migrant workers have been a huge and indispensable group. As the outstanding and important force of constructing China's well-off society, migrant workers have created the new economy model honored as"labor economy"to solve the three dimensinonal rural issues and they have injected new vigor for the urban development. However, influenced long by the barrier of urban-rural, the two-element system and the accumulation of two-element economic construction, two-element social construction and two-element social guarantee system caused by this barrier system, migrant workers'work and lives haven't got rid of the bound of two-element system. Long work time, high- intensity work and tatty life conditions weak migrant workers'resistance ability to disease to large extent, and their health status is worrying. They have become the group in worst need medical service."The persons in worst need medical treatment are often those who can't afford medical expense."Facing the unstable work and limited income, it is expensive and difficult for migrant workers to see doctors, which seems particularly prominent. They need badly to obtain the medical insurance so as to alleviate their economic burden, and guarantee the accessibility of their basic medical service. Based on the importance of migrant workers'problem and their urgent demand for medical insurance, the study on the problem of migrant workers'medical insurance has t great theoretical significance and practical meaning in guaranteeing migrant workers'basic health right, perfecting medical insurance system and furthermore promoting the construction of harmonious society.In this thesis, under the guidance of related theory of migrant workers'medical insurance, using theoretical analysis and empirical analysis, we analyze the present situation of our country's medical insurance and existing problems, then combine with development tendency of migrant workers and the present resources of two-element medical insurance system to design the general model construction of migrant workers'medical insurance. At last, from two aspects of fund operation mechanism and system environment optimization, we describe the safeguard measures that ensure the benign operation of migrant workers'medical insurance hope to provide the theory basis and policy reference for the construction of migrant workers'medical insurance system and the final formation of urban and rural one-element medical insurance system.This thesis consists of seven chapters.Chapter One Introduction In this chapter, we mainly describe the study purpose and significance of the thesis, briefly review the study dynamics at home and abroad, establish the study thought and methods of this thesis and summarize the possible innovation.Chapter Two The theoretical foundation of peasant workers'medical insurance study. This chapter is theoretical foundation of the subsequent study in this thesis. We interpret and define the main concepts concerning the thesis"migrant workers"and"medical insurance", which are very common and disputed concepts in correlated study field of economics and sociology, thus, establish the connotation of migrant workers'medical insurance. Then, we discuss manpower capital theory, social guarantee system, social fair theory, public product theory, health economics theory and the basic theory of insurance, which provide the theoretical support for studying peasant workers'medical insurance.In Chapter Three, we analyze migrant workers'medical insurance situation under binary medical system. We discuss in detail the development and basic system framework, and point that insulation and localization management of binary medical insurance system lead to the deficiency of migrant workers'basic medical insurance. Consequently, we should establish the medical insurance which can suit instability of migrant workers.In Chapter Four, we talk about the realistic basis and feasibility of founding migrant workers'medical insurance. Under the condition of simple and unsafe working and living situation and limited income, migrant workers'medical insurance is an urgent task. We analyze the government's willingness, financial capability, and executive ability, enterprises'endurance and migrant workers'individual willingness and payment ability, and prove the feasibility of constructing migrant workers'medical insurance general model.Chapter Five centers on construction of general pattern of migrant workers'medical insurance. Based on analysis and evaluation of current migrant workers'medical insurance region model operation's effects, we form general idea and establish basic rules of constructing migrant workers'medical insurance general model. Under the guidance of the general idea and rules, we establish the area of migrant workers'medical insurance, study and design the charge, account, and payment of migrant medical insurance, and discuss the relation and connection between migrant workers'medical insurance and current binary medical insurance system. In Chapter Six, we talk about the design of migrant workers'medical insurance funds operation mechanism. Starting from raising funds of migrant workers'medical insurance, we give a detailed analysis of migrant workers'medical insurance funds'management model, the choice of investment methods, and financial accounting systems to ensure increment and maintenance of migrant workers'medical insurance funds.In Chapter Seven, we focus on the optimization of migrant workers'medical insurance system. Looking at four aspects of ensuring migrant workers'equal right of employment, enhancing enterprises'undertaking social responsibility, exerting trade union's right protection mechanism and weakening household register system, we put forwards the measures that can optimize the external environment of operation of migrant workers'medical insurance systems.
