Cerebral Hemodynamics In Hypertensive Patients, A Transcranial Doppler Study

OBJECTIVE: 1 .To compare the sensitivities of using blood velocity and the rate of artery abnormality obtained from Transcranial Doppler (RAATCD) to the change features of cerebral arteries in primary hypertensive patients. 2. To analyze the correlations of RAATCD and age, duration of hypertension and some other risk factors for stroke.METHODS: Two hundred and fifty-four primary hypertensive patients without stroke or stroke history, diabetes mellitus or other intracranial diseases were included in this study. Vp, the main parameter of blood velocities and RAATCD from these patients was calculated. Thereafter, the correlations of Vp or RAATCD to the age, duration of hypertension and some other risk factors for stroke were analyzed with SPSS 10.0, respectively.RESULTS: 1. No correlations between mean Vp and age, duration of hypertension. 2.Patients with age older and duration of hypertension longer, their RAATCD appeared higher, and flow velocities of MCA and ICA increased significantly. VA was influenced at the early stage of hypertension. After 10 years of hypertension, RAATCD of ICA systems increased prominently. RAATCD of each cerebral artery was linear correlated with duration of hypertension. Both age and duration of hypertension were risk factors for stroke, but the duration of hypertension was more correlated. 3. The hemodynamic changes obtained from TCD were in accord with the features of stroke patients. 4. Hypertensive subjects were always companied with hyperlipidemia and hyperuriemia. RAATCD was significantly correlated with TG.CONCLUSIONS: RAATCD was a more sensitive parameter related to the cerebral arteries changes in hypertensive patients. Age, the duration of hypertension and triglyceride were independent risk factors for stroke. The hemodynamic changes obtained from TCD, being inaccord with the features of stroke patients, might be an independent stroke risk factors for hypertensive patients.
