Objective: Transforming growth factor-β (TGF-β),nitric oxide (NO) and angiotensin II (AngH) are the important active factors derived from the endothelial cells. They have closely relationship with the ventricle remodeling processes, myocardial hypertrophy and renal fibrosis. TGF-β and Anglican accelerate the collagen synthesis while NO can promote degradation of collagen being an endothelial cell-depended diastolic factor. The forming of deposition of extracellular matrix in kidney and even the renal fibrosis were occurred caused by the unbalance of the regulators of synthesis and degradation of collagen. The molecular mechanism in hypertensive kidney lesion was demonstrated by monitoring the levels of serum TGF-β, nitric oxide and plasma AngH in essential hypertensive patients and investigating the effect of information convection in endothelial cell on fibrillar collagen metabolism and its relationship with hypertensive kidney lesion. Methods: 101 patients with essential hypertension were divided into two groups according to urine microalbuminuria: with and without kidney lesion. Among them, 46 patients with kidney lesion were divided again into two sub-groups: subclinical group with such characteristics as positive in urine microalbuminuria, negative in urine regular protein and without kidney lesion, while the clinical one with the characteristics like both positive in urine microalbuminuria and in urine regular protein, with kidney lesion. 61healthy persons were selected as healthy group. The serum concentration of TGF-β1 was measured by ELISA method; nitre oxide was measured by nitrit reductase method, plasma Ang II was measured by radioimmunoassay. Results: (1) The levels of serum TGF-β1 , nitre oxide and plasma Ang II were significantly difference among hypertensive patients with and without kidney lesion and healthy control(p<0.05).(2) There were also significantly difference of TGF-β1, nitre oxide and plasma Ang IIbetween sub-clinical group and clinical one, (The t values were 4.10; 2.16 and 2.10 respectively, p<0.05). (3)There were some relationship among TGF-β, , NO and Ang II (r were 0.5367 and -0.5852 respectively, P<0. 05). Conclusions: (1) TGF-β1, nitric oxide and AngII are involved in the metabolism of extracellular matrix in hypertensive kidney, and act as important roles in the occurrence and development of hypertensive kidney lesion. (2) Vascular endothelial dysfunction and the disorder metabolism of collagen may be one of the episode mechanism in hypertensive kidney lesion.Ma Hong (Cardiology) Directed by Zhang Qi yi...
