Objective: Dyspepsia refers to the syndrome including ache or unease in epigastric , abdominal distension, belching, anorexia, nausea, vomit and so on. It is the most common clinical manifestation for gastroenterologist. It can be caused by systemic or local organic diseases, called organic dyspepsia. The prevalence of hepatitis, hepatic fibrosis, hepatic cirrhosis and some other liver diseases such as fatty liver, alcohol liver disease is very high in our country. The insufficient gastrointestinal function such as motility, absorption, barrier, circulation caused by liver diseases is called hepatic gastrointestinal insufficiency. Many patients with liver diseases go to hospital because of dyspepsia at the first time. There are also a large number of patients without any organic disease, called Functional dyspepsia(FD) . The pathogenesis of FD is not illustrated entirely.Enteron motility insufficiency in digestive and interdigestive periods is present in about 30-80% patients with dyspepsia. Gastric emptying function is an important index to evaluate gastric function in digestive period. The pathogenesis of dyspepsia is related to many factorsincluding nerve system, endocrine, psychology and so on. The study about influence of gastrointestinal hormone is not sufficient. Our study is to investigate the relationship between gastrointestinal hormone and dyspepsia.At present, prokinetic agents are being applied to treat dyspepsia, such as Domperidone, Cisapride. A part of patients can be improved, but they do not work on many people. We use Qingre Liqi Decoction to treat patients with hepatic dyspepsia and FD. Comparing the difference of gastric emptying, gastric hormone, symptom score between before and after treatment, we evaluate the clinical effect of this medicine.Methods: 55 patients with dyspepsia were allocated to chronic hepatic group(n=27,m 16,f 11)and FD group(n=28,m 13,f15). The control group (n=28,m 8,f 10) was healthy adult. The 3 groups all accepted gastric fluid emptying test. Their half gastric emptying time is recorded, venous blood is collected after fast for 8﹣12 hours. The blood Gastro- intestinal Hormone level including plasm vasoactive intestinal peptide (VIP), cholecystokinin (CCK), motilin (MTL), serum gastrin (GAS) was measured. Their symptom score were calculated. The difference of half gastric emptying time and hormone level between the dyspepsia groups and the control group was observed. The patients were treated with Qingre Liqi Decoction at the dose of 125ml , twice a day. Gastric motility, hormone level andsymptom score were investigated again after 5 days in both dyspeptic groups. To evaluate the clinical effect of Qingre Liqi Decoction, we also analyzed the correlation between symptom score and gastric emptying time, gastrointestinal hormone.Results: There were significant differences of gastric emptying time between dyspepsia groups and control group (hepatic group 16.52±9.44min ; FD group 17.04±7.57min;control group 10.66±2.30min p<0.01)。The difference of hormone level between hepatic group before treatment and control group: There was no significant difference on MTL (p>0.05), The GAS,VIP and CCK level was obviously higher than those in control group (p<0.05); The difference of hormone level between FD group before treatment and control group: The MTL level of FD group was obviously lower than control group (p<0.05), GAS and CCK level was evidently higher than control group (p<0.05), There was no significant difference about VIP level; The difference about gastric emptying , symptom score and blood gastrointestinal hormone before and after treatment: The symptom score : Hepatic group turned from 9.58±3.44 to 7.04±3.10, FD group turned from 9.16±3.66 to 6.48±3.23. The differences were all significant (p<0.001). The half gastric emptying time: Hepatic group turned from 16.52±9.44min to 10.63±4.50min , FD group turned from 17.04±7.57 to 12.78±4.53min.
