Objective:To detect protein expression of PTTG and CD34 in pituitary adenomas and explore the relationship between PTTG expression and angiogenesis labelled with anti-CD34 monoclonal antibody and invasiveness of pituitary adenomas and investigate its clinical value. Methods:44 pituitary adenomas specimens removed in our department from May,1998 to July,2001 and 3 normal pituitary tissue were collected ,all fixed with 4% formalin and then paraffin embedded.Every case history was detailedly reviewed,and all specimens were divided into invasive group(35 cases) and noninvasive group(9 cases) by combindedly adopting Knosp imaging diagnosis standard and Renzuyuan's standard and consulting all operation records.Expression of PTTG protein and angiogenesis status (counted by MVD)labelled with anti-CD34 monoclonal antibody in 47 slices of specimens were examined by immunohistochemistry.Finally we analyzed the relation between expression of PTTG protein and angiogenesis status and invasiveness of the pituitary adenomas,and discuss the correlation between expression of PTTG protein and angiogenesis status.The correlation between colligation therapy method and curative effects of the invasive pituitary adenomas were also analyzed.Results:1.Immunohistochemistry experiment shows that the PTTG protein was mainly distributed in cytoplasm and membrane of tumor cell,and positive staining is yellow grain.There is higher PTTG protein expression in invasive group and lower in noninvasive group and normal pituitary tissue dose not express it.There is a significant correlation between expression level of PTTGprotein and invasiveness of the pituitary adenomas(P﹤0.001).Idenfication standard of positive staining of the CD34 protein:any single brown-stained cell or cluster of endothelial cells that were clearly separated from the adjancent microvessel,tumor cells and other connective tissue elements were considered as a vessel.Higher angiogenesis is detected in the invasive group and lower in noninvasive group and normal pituitary tissue.There is a significant correlation between angiogenesis status and invasiveness of the pituitary adenomas(P﹤0.001).There is not a significant difference of angiogenesis status between noninvasive group and normal pituitary tissue. There is a significant correlation between angiogenesis status and expression level of PTTG protein(r=0.795, P﹤0.001).2.The statistical analysis of clinic information indicates a significant relationship between invasiveness of the pituitary adenomas and tumor size,excise ratio, curative effects(P﹤0.05);the curative effects of operation plus radiation therapy grouping excel the simple operation grouping in the invasive group(P﹤0.05).Conclusions: 1.Higher protooncogene PTTG protein expression and tumor angiogenesis labelled with anti-CD34 monoclonal antibody may play important roles in the invasiveness of pituitary adenomas,so we think they can serve as molecule markers of adenomas invasiveness.2.Colligation therapy method should be adopted to improve curative effects of the invasive pituitary adenomas,such as microneurosurgery operation, auxiliary postoperation radiation therapy,γ-knife treatment, anti- angiogenesis and immunity gene therapy...
