Objectives: 1. To probe the clinical significance of the changes of heart rate variability (HRV) in patients with chronic heart failure (CHF). 2. To observe the relationship between the changes of HRV and the degrees of cardiac function, the primary affections, the types of CHF and zheng differentiation-classifications. 3. To investigate the changes of HRV after treatment in controlled group and traditional Chinese drug treatment (TCDT) group, and the differences between them.Methods: 1. 76 cases with CHF were divided into controlled group and TCDT group randomly, 38 cases per group. Controlled group was treated by regular western medicines, and TCDT group treated by Kangdaxin oral liquid added to regular western medicine. 2. The degrees of cardiac function, the primary affections, zheng differentiation-classifications, the types of CHF, and the HRV of all case with CHF were recorded after and before treatment. 3. 24 healthy subjects served as normal group.Results: 1. The HRV (SDNN, SDANN) in patients with CHF was lower than that in normal subjects (P<0.05 or 0.01). The more severe CHF was; the more lower the HRV(P<0.01 in all). 2. The differences of HRV in patients with CHF were not significant among different primary affections(P>0.05), but significant among different types of CHF (P<0.01). The indexes (SDNN, SDANN) of the HRV in patients with whole CHF were significantly lower than those in patients with left or right CHF(P<0.05 or 0.01). 3. The indexes (SDNN, SDANN) of the HRV in patients with different zheng differentiation-classifications show gradually falling trend as following: Xinqiyangxugroup> Qixuxueyu group> Xinshenyangxu group>Yangxushuifan group>Xinyangxutuo group. There were remarkable differences among them as a whole (P<0.01), but not all was so. 4. The indexes (SDNN, SDANN) of HRV in patients with CHF of TCDT were raised after treatment, and the differences were remarkable (P<0.05), while those of the controlled were not. 5. The effect of TCDT on the indexes (SDANN, SDNNI) of HRV of CHF was better than that of regular western medicine (P<0.05 or 0.01).Conclusions: 1. The HRV can be used to judge the severity of CHF. 2. There was no relationship probably between the changes of all indexes of HRV and different primary affections; the changes of the indexes (SDANN, SDNNI) of HRV of CHF were remarkable among different types of CHF. 3. The changes of these indexes (SDNN, SDANN) contribute to districting different zheng differentiation-classifications. 4. The effect of TCDT added to regular western medicine was better than that of regular western medical treatment.
