Experimental Study On Evaluation Indexes For Therapeutic Effects Of Postoperative Adhesions

Objective: (1) To observe the effect of Changtong Oral Liquid (CTOL) on postoperative adhesions; (2) to determine and analyse the number of white blood cell (WBC), contents of fibrinogen(FIB), tumor necrosis factor alpha (TNF-a), transforming growth factor β1(TGF-β1), interleukin-1β (IL-1β) interleukin-4 6 10(IL-4 IL-6 IL-10) in the blood of rats with adhesions; (3) try to study the influence of WBC FIB and cytokines on postoperative adhesion formation and to pick out the significant indexes of evaluating postsurgery adhesions so as to create an objective accurate and reliable index system for estimating clinical therapeutic effects.Method: (1) Fifty-four Sprague-Dawley (SD) male rats were used in this study and randomly divided into six equal groups (N= 9): normal control (group 1); model control (group 2); SMT (group 3); and CTOL (group 4, 5 6). Except for group 1, the other rats were created as intestinal adhesive models with the guidance by Ellis. Normal and model control groups were orally supplemented with distilled water; SMT used as male control medication showed similar efficacy as CTOL to assure the reliability of experimental method. Experimental drug was allocated to three different dosages of 4.3g/kg, 8.6 g/kg, 17.2 g/kg respectively, which were 5, 10, 20times as much as clinical dosage. The dose of male control medication wasequal to the time of CTOL medium dosage. On Day 7 after surgery the adhesions were graded; additionally, blood was drawn from abdominal artery from all rats to determine the amount of WBC, concentrations of FIB and levels of TNF-a IL-1β TGF-β1, IL-4, IL-6, IL-10.(2) Ninety SD male rats were randomly allocated to two groups. Intestinal adhesive models were made in accordance with Ellis. The blood samples of CTOL medium dosage and model control group were collected on the 1st, 3rd, 5th, 7th, 9th day after surgery ; Nine rats were killed after their blood was drawn at different times from each group. At last, serum levels of TNF-a TGF-β1 IL-1β IL-4. IL-6 IL-10 were tested using enzyme-linked immunosorbent assay kits.Results: CTOL can evidently prevent rat intestinal adhesive models, reduce the severity of postoperative adhesions, and improve the pathological injuries of adhesive spots; CTOL can also decrease the WBC number and contents of FIB in the plasm and serum levels of proinflammatory cytokines, such as TNF-a IL-1β TGF-β1 IL-6. However, it has no significant impact on serum levels of anti-inflammatory cytokines IL-4,IL-10 of postsurgery adhesive rats.Conclusion: CTOL can remarkably improve the severity of postoperative adhesions. This experiment has selected significant indexes for assessing adhesions after operation, such as WBC, FIB and cytokines (TNF-a, IL-1β, TGF-β1 IL-6), and provided experimental basis for evaluation clinical therapeutic effects of postoperative adhesions and drug study to establish objective exact and reliable evaluating indexes of clinical therapeutic effects.
