Primary Pancreatic Carcinoma (PPC) as a common malignant tumor in alimentary , it is very difficulty to be cured , and it has increasing incidence of disease. PPC is difficulty to be detected in early grade , it can invade and metastasize in early grade easily , it lacks effective methods of theraphy . Many researches showed that heparanase (Hpa) has a close relationship with development and metastasis of carcinomas . Hpa is endo- -glucuronidases , it plays an important role in tumor's growth , invasion, metastasis and angiogenesis , and MVD is a reliable indication to evaluate angiogenesis of tumors .The research of MVD and expression , clinical significance of Hpa2 in PPC is not reported . Objective:In this research , MVD and expression of Hpa2 were detected in 26 PPC specimens by immunohistochemistry to explore the relationship of Hpa2 and MVD with PPC and its clinical significance . Material and Methods :Specimens of PPC were obtained from 26 patients who underwentsurgery .There were 10 males and 16 females ,The age of patients arranged from 34 years to 74 years (mean: 54.3 years).The diameter of tumor arranged from 0.5cm to 5cm , and there were 5 patients with the diameter of tumor less than 2cm, and 21 patients with the diameter of tumor more than 2cm .The cases were divided into four clinical grades according to the criteria discribed by UICC in 1997: grade I 2 cases; grade II 7 cases; gradelll 15 cases; gradelV 2 cases .The normal pancreatic tissue resected from 2 patients with lacerating injury of pancreas were used as normal control . Anti-Hpa2 polyclonal antibody and Anti-CD34 monoclonal antibody were used to perform immunohistochemistry and the specimens were stained with SABC method. When the positive parenchymal is more than 11% of the total cells in random high power field, the expression of Hpa2 was regarded as positive. MVD was indicated by mean number of vessels of three high power fields .Statistical analysis was executed by SPSS10.0 software, a value less than 0.05 was considered as significance. Results :1. The expression of Hpa2 was negative in normal control group .2. The positive rate of Hpa2 expression was 69.23%(18/26) in PPC, and that was 33.33% (7/21) in paracarcinoma tissues , and there was significant difference between two groups (P<0.05) .3. The positive expression of Hpa2 had no relationship with the age -, sex of patients and the diameter of tumors (P>0.05) . The positive rate of Hpa2 was 33.33%(3/9) in low clinical grade group (grade I +11), and that was 82.35%(14/17) in high clinical grade group (gradelll+IV), and there was a significant difference between two groups (P<0.05) . The positive rate of Hpa2 in the group with intact capsule was 40.00%(4/10),and that was 87.50%(14/16) in the group without intactcapsule ,and there was a significant difference between two groups (P<0.05) . The positive rate of the group with metastatic lymphaden was 88.89%(16/18) , and that was 37.50%(3/8) in the group without metastatic lymphaden , and there was a significant difference between two groups (P<0.05 ) .4. The MVD had no relationship with the age , the sex of patients and the diameter of tumor (P>0.05).But the MVD was15.92 4.26/HP in low clinical grade group (grade I +11 ), and that was 21.34 4.62 /HP in high clinical grade group (grade HI + IV ) ,and there was a significant difference between two groups(P<0.05) .The MVD was 16.41 4.16 /HP in the group with intact capsule, and that was 21.38+4.84/HP in the group without intact capsule, and there was a significant difference between two groups (P<0.05 ). The MVD was 21.06?.71 /HP in the group with metastatic lymphaden , and that was 15.88 4.36/HP in the group without metastatic lymphaden , and there was a significant difference between two groups (P<0.05) .5. The MVD was 20.84+4.99 /HP in Hpa2 positive group and that was 16.39 + 4.26/HP in Hpa2 negative group, there was a significant difference between two groups (P<0.05) .Conclusions:1.
