Objective: To study the expression and significance of TRAIL(TNF-related apoptosis-inducing ligand), DR4,DcR1, Survivin,NF-κB and Caspase3 in colorectal carcinoma.To investigate the effect of DR4,DcR1,Survivin, NF-κB and Caspase3 on the anti-tumor activity of TRAIL and the apoptosis mechanism induced by TRAIL and seek the pathway to increase the sensitivity of anti-tumor effect by TRAIL. Methods: S-P immunohistochemical technique was used to examine the expression of TRAIL, DR4,DcR1,Survivin, NF-κB and Caspase3 in 42 cases of colorectal carcinoma and the surrounding tissues of 5cm distance and 25 cases of normal colorectal mucosa. The level of the expression was analyzed combined with clinicopathological features of age and sex of the cases, the size and loci of the tumor, infiltration depth, differentiation degree, naked eye and histology type, lymph node metastasis and Dukes-stages. Results:The color of the immune positive products of TRAIL, DR4, DcR1, Survivin, NF-κB and Caspase3 was buffy and located in cytoplasm or plasmalemma except NF-κB was in nucleus. The rate of positive expression of TRAIL was 23.81%(10/42), 59.52%(25/42) and 84.00%(21/25) in colorectal carcinoma, surrounding tissues and normal colorectal mucosa respectively and the tendency of increase was obvious(P<0.01). The rate of positive expression of DR4 was 47.62%(20/42), 71.43%(30/42) and 80.00%(20/25)in colorectal carcinoma, surrounding tissues and normal colorectal mucosa respectively. The expression of DR4 was decreased in colorectal carcinoma and has a significant difference with surrounding tissues and normal colorectal mucosa(P<0.05). The rate of positive expression of DcR1 was 71.43%(30/42),26.19%(11/42) and 24.00%(6/25)in colorectal carcinoma, surrounding tissues and normal colorectal mucosa respectively. The expression of DcR1 was increased in colorectal carcinoma and has a significant differences with surrounding tissues and normal colorectal mucosa(P<0.01). The rate of positive expression of Survivin was 59.52%(25/42), 4.76%(2/42) and 0.00% (0/25) in colorectal carcinoma, surrounding tissues and normal colorectal mucosa respectively. The expression of Survivin was increased in colorectal carcinoma and has a significant difference with surrounding tissues and normal colorectal mucosa (P<0.01). The rate of positive expression of NF-κB was 54.76% (23/42), 28.57%(12/42) and 16.00%(4/25) in colorectal carcinoma, surrounding tissues and normal colorectal mucosa respectively. The expression of NF-κB was increased in colorectal carcinoma and has a significant differences with surrounding tissues and normal colorectal mucosa(P<0.01). The rate ofpositive expression of Caspase3 was 47.62%(20/42), 83.33% (35/42) and 92.00%(23/25) in colorectal carcinoma, surrounding tissues and normal colorectal mucosa respectively. The expression of Caspase3 was decreased in colorectal carcinoma and has a significant differences with surrounding tissues and normal colorectal mucosa(P<0.01). The expression of TRAIL was lower in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression was 5%(1/20) and 40.90%(9/22) respectively (P<0.01). The expression of DR4 was lower in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression was 20.00% (4/20) and 72.73% (16/22) respectively (P<0.01). The expression of DcR1 was higher in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression was 95.00%(19/20) and 50.00% (11/22) respectively. The expression of Survivin was higher in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression was 80.00% (16/20) and 40.91% (9/22) respectively (P<0.01).
