Background: The succeed of the heart transplantation is principally depended on the use of the cyclosporine A (CsA), which restrains the activeness of the TH cells and the lymphocyte, inhibits lymphocyte proliferation and interleukin-2 expression. But at the same time , the side effect of the CsA is remarkable , for example ,the toxicity on the kindy and the liver. So ,we have to give our attention to some drugs which not only can protect the cardiac myocytes, but also can enhance the ability to resist the infection and immunity of the inplant or lighten the side effect of the immunosuppressant. For instance theFructose-1,6-diphosphate (FDP).The ischemia-reperflision injury (IRI) during the heart transplantation is a pivotal factor for the acute rejection and the succedent chronic rejection. So ,the interferential factors to abate IRI are protective formyocardium on maintaining the framework and advancing the function . It has been approved that FDP has markable curative effect on the small bowel after transplantation in rats. At the same time , some foreign scholar have investigated if FDP has the ability on immunity (no record to today ). During this research, we base on the the model of cardiac allograft in rat, using FDP solely and combining with CsA, take stock of the difference with the group control , we get the data of the groups, for instance the self-recovery time and survival time , evaluate the mechanism of the FDP to the implant. As a result , the effect of the FDP and CsA on implant is different but they go into effect during peritransplant is harmonious . At civil, there still no written report on this aspect.Objective: FDP can alleviate the IRI during the heart transplantation ,which has the ability to stability the cell membrane > restrain the oxygen free radical and cell apoptosis of the myocardium. The CsA, as a mature and positive immunosuppressant , has been used widely. There is still no verdict if the FDP can be used as a immunosuppressant up to the present, but the action for the cells is clear. In this research ,we use FDP solely and combine with CsA on the the model of cardiac allograft in rats, observe the effect on self-recovery and survival time of the replanted heart, get the ultimateness that the FDP is effective on maintaining myocardium cytoarchitecture, abating IRI, prolonging the survival time of the donor's heart ,all of these proved that itcan be used during peritransplant in clinic, to advance the livability of the implant.Methods : Clear Wistar rats and SD rats , each 20 , 4-week-old, average weight 250-300g ,are be used to be donors and recipients respectively, divided into 4 groups randomly : control (n=10), FDP (n=10, 350mg/(kg.d)), CsA(n=10,5 mg/ (kg .d) ), FDP+CsA(n=10, FDP 350 mg/ (kg.d) +CsA5mg/ (kg.d) ).The anaesthesia is Ketamine 150 mg/Kg+Diazepam 2.5mg/Kg through ventrolateral injection;Transplantable method is obey to modified Ono' s operation: Firstly, the recipient is anaesthetic and breathe with oxygen, open the abdomen to separate the celiac artery and vena cava, succedent covered with axenic gauzes. Secondly, the donor received anaesthesia and tracheotomy, assistant breath with ventilator ,we open the chest and heparinize the rat from the vena cava, affuse the 4°C cardioplegia from ascent part of the main artery, place the ice on the surface of the heart to slow the HR and reduce CO, after the heart stop pulsation, take down it and repair in Ringer's solution. The donor's main artery and pulmonary artery are anastomosed with the recipient's celiac artery and vena cava respectively. The replanted heart's coronary artery is perfused from recipient's celiac artery, which help it self-recovery. After hemostasia ,we suture the incision, preserve animal heat till the rat wake up , Self-recovery and survival time of the replantedheart were collected.The successful symbol of the operation is the recipient and donor's heart live out exceed 24 hours. Look-in the self-recovery and survival time of the four groups. Data were expressed as means±SEM, and analysised by the SPSSll.Ol, assume P^0.05 is the standard of statistical significance.Results:(1) average self-recovery time of the donor's heart xontrol group:(9.6±1.34) seconds, FDP : (5.0±0.70)s; CsA: (10.2±0.84)s, FDP+CsA: (4.8±0.45)s.(2) average survival time of the donor's heart: control group(6.0±1.00)days; FDP: (12.4±1.14)d; CsA: (11.6±1.14)d; FDP+CsA: (18.0±1.00)d.On self-recovery time the group FDP and FDP+CsA had a significantly shorter duration than the control group (P<0.05) . As compared to the control group , the three experimental groups had significant difference on survival time (P<0.05), especially the group FDP+CsA (P<0.01) ,but there was no difference between group FDP and CsA.Conclusion: Fructose-1,6-diphosphate is a very important mesne production in energy metabolizability and has the ability to stability the cell membrane > restrain the oxygen free radical and cell apoptosis.lt can promote donor's heart recovery through abating ischemia-reperfusion...
