| BackgroundAdhesion is a difficult clinical problem after allograft of tendon. Research-ers all over the world try to resolve it by many methods , but no progress is a-chieved. Now,people believe that allograft tendon have intrinsic and extrinsic healing, and the proliferation of the peripheral connective tissue cause the adhesion. Thus, our experiment try to prevent adhesion by the combined slow-releasing degradatable membrane . The membrane consist of bFGF and VitaminC, which can increase the amount of fibroblast and improve collagen fibri synthesis.ObjectiveTo study the adhesion-preventing effect on the allograft tendon healing by the combined slow-releasing degradatable membrane, consists of bFGF, which can increase the amount of fibroblast inside the tendon, and VitaminC, which can improve collagen fibrils synthesis.MethodsThe bFGF combined slow-releasing degradatable membrane is made of bFGF and VitaminC . Thirty New Zealand white rabbits , weight 2.0 ~ 2. 5kg(a-chieved from CMU) , were divided into A, B two groups randomly with each group contains fifteen rabbits .After anesthesia, 2cm achilles tendon of the left posterior limb is excised , and allografts of Achilles tendon deep frozen by -196℃ were sutured with approved Kessler method. Group A : sutured achillestendon were encapsulated with bFGF combined slow-releasing degradatable membrane ; Group B : sutured achilles tendon were encapsulated with degradatable membrane without any agent . After operation,the rabbits were fixed with plaster cast and given routine therapy. 14N30>60days later, light-microscope,e-lectron-microscope,figure-analysing,extend of peritendon adhesion were evaluated.Results1. It was shown on the HE,Mallory and AgNO3 stained specimen that the a-mount of fibroblast , collagen fibrils and the vitamin-C particles inside the al-lografted tendon is larger than that of peripheral connective tissue and group B (p <0. 01). Under the electron-microscope, the amount of excellent fibroblast and collagen fibrilla in group A is larger than that of group B .2. The peripheral tissue in group A almost remains the formal loose connective tissue and it seldom grows into tendon. There is a clear border between the tendon and the peripheral tissue. But, it becomes dense connective tissue in group B, and grows into tendon. The amount of fibroblast neucleus and collagen fibrilla in peripheral tissue is larger than that in the tendon.3. Moreover,the extent of adhesion in group A is lesser than B group according to the mensuration of peritendon adhesion ( P <0.01).ConclusionTwo ways exist in allograftd tendon repair , named intrinsic and extrinsic healing . In the healing of allografted tendon, it is the peripheral tissue which grow into the tendon and caused the adhesion. The bFGF combined slow-releasing degradatable membrane can make the intrinsic healing of tendon faster than peripheral connective tissue proliferation , so it can prevent the formation of adhesion. |
