Objective: To investigate the role of IGF-1, IGFBP-1 and Ieptin in the genesis of hypertensive disorder complicating pregnancy, and the relationship between these cytokines and the disease, so as to provide some scientific evidences for clinical monitoring, evaluation and interventions. Methods: Maternal serum IGF-1 , IGFBP-1 and Ieptin were measured by immunoradiometric assay in 38 women with hypertensive disorder complicating pregnancy including 11 cases with gestational hypertension and 27 cases with pre-eclampsia and 38 normal pregnant women, matched for maternal age body mass index and gestational age at sampling. The correlations between the serum IGF-1 , IGFBP-1 and Ieptin levels and blood pressure, BMI at sampling, and neonatal birth weight were analyzed by the correlative analysis. Results: In patients with hypertensive disorder complicating pregnancy, serum levels of IGF-1 were lower, and IGFBP-1 were higher, respectively, than those of control group(P<0.01). The leptin concentrations were not significantly different among groups with gestational hypertension ,pre-eclampsia and normal pregnant. In pre-eclampsia group, the levels of IGF-1 were lower and IGFBP-1 were higher, respectively, than those of control group(PO.Ol) and gestational hypertension (P<0.01) There were no significant differences between the gestational hypertension and control group in the levels of IGF-1 and IGFBP-1.The levels of leptin were positively correlated with BMI but not correlated with blood pressure. There were negative correlation between IGF-1 and systolic blood pressure , diastolic blood pressure and mean arterial pressure respectively, and positive correlation between IGF-1 and neonatal birth weight. There were positive correlation between IGFBP-1 and systolic blood pressure , diastolic blood pressure and mean arterial pressurerespectively, and negative correlation between IGFBP-1 and neonatal birth weight. Conclusion: Decreased serum IGF-1 levels , increased values of IGFBP-1 with blood pressures suggested that the cytokines may play a role in the occurrence and development in hypertensive disorder complicating pregnancy, the relationship between the aboved cytokines and neonatal birth weight suggested that IGF-1 and IGFBP-1 may be the predictors of pregnancy outcomes in hypertensive disorder complicating pregnancy. The role of leptin in hypertensive disorder complicating pregnancy needs be further explored.
