[ Objective ] : To study the effect of oxygen free radicals(OFR) and antioxidation enzymes in the pathogenesis of varicose veins of lower extremity by measuring the malonyldialdehyde(MDA) and super oxide dismutase(SOD) in the varicose vein walls of two groups of patients classified according to the state of ailment.[ Material and Methods ] : We collected 95 vein specimens of long saphenous vein (LSV) and distal shank from 40 patients who underwent varicose vein surgery at Qianfoshan hospital of Shandong province during the period from May 2004 to November 2004.The patients were 7 males and 33 females with an average age of 46.9±SD8.7years(32-70years).We classified the patients into group A and group B according to the criteria followed:(1) longer course of disease((?) 10 years);(2)feeling heaviness, pain in the limb with varicose veins; (3)nutrition changing or ulcerating of skin at ankle. Vein specimens of patients conforming with two or more than two items,or conforming with only the third item were divided into group B,including 43 specimens(24 middle-thigh LSV specimens marked with B1 and 19 distal LSV specimens marked with B2 ) ,and others into group A, including 52 specimens(26 middle-thigh LSV specimens marked with A1 and 26 distal LSV specimens marked with A2 ).None of the patients have a history of thombophlebitis or injection sclerotherapy in the past.During surgery,vein specimens were separately collected from both the mid-thigh LSV and distal LSV.The vein specimens were washed in normal saline,immidiatly placed inseparate test tubes and frozen at -20 °C,until the time of processing.We measured albumen, SOD and MDA using specific reagent kit and processed strictly according to the certification. SPSS 10.0 statistically processed the data. [Result] :1, We measured MDA in the wall of 52 vein specimens of group A and got an average concentration of 0.27nmol/mgprot in group Al and 0.50nmol/mgprot in group A2.The difference between group Al and group A2 is statistically significative(/?<0.05).MDA concentration was also measured in the wall of 51 vein specimens of group B.The average MDA concentration of group Bl is 0.33 + 0.04 nmol/mgprot,which is just one third of that group B2(0.98 + 0.1 0nmol/mgprot).The data was analyzed by SPSS10.0 and the difference between group Bl and group B2 was found statistically significant (p<0.01).2^ We measured SOD activity in the wall of 52 vein specimens of group A and got an average SOD of 4.43 ± 0.29U/mgprot in group Al and 6.06 + 0.48U/mgprot in group A2.The difference between group Al and group A2 is statistically significative (p<0.05). SOD activity was also measured in the wall of 51 vein specimens of group B.The average SOD of group B2 is 7.14 ±1.33 U/mgprot,which is 1.3 times that of group Bl(5.46 + 0.56 U/mgprot).The data was also analyzed by SPSS10.0 and the difference between group Bl and group B2 was found statistically significant (p<0.05).3^ The MDA concentration of the vein specimen walls of the same area in group A and group B was compared.The result shows that the MDA concentration of group Bl is distinctly higher than that of group Al and is 1.22 times the latter.The difference between the two groups is statistically significant(p<0.05).The analysis also shows that the MDA concentration of group B2 is obviously higher than that of group A2,which is about one secondthe former.The difference between group.B2 and group A2 is also statistically significant (/?<0.05) .4 > The average SOD activity of the vein specimen walls of the same area in group A and group B was also compared.The result shows that the average SOD activity of group Bl is obviously higher than that of group Al .The difference between the two groups is statistically significant(p<0.05).The analysis also shows that the average SOD activity of group B2 is higher than that of group A2.The difference between group B2 and group A2 is also statistically significant (p<0.05) .[Conclusion] :The result of this study can lead us into some important conclusions. First, the fact that patients suffering from serious varicose veins of lower limb have a higher average MDA concentration than patients with comparatively light illness does, indicates that enhanced peroxide reaction induced by OFRs is closely related with the progress of the disease. Second, the average MDA concentration in the walls of varicose veins from distal shank is higher than that from mid-thigh LSV, and the extent seems to be growing with the progress of the disease; Third, althouth the SOD level rises with the increase of MDA, the extent does not matchs well, which indicates a deficiency of antioxide substance in patients suffering from varicose veins of lower limb.Based on these findings, we can conclude there is a close relationship between peroxide reaction induced by OFRs, relative insufficiency of SOD and the development of varicose veins of lower limb.
