Observation Of The Efficacy Of Narrow-band Ultraviolet B Phototherapy In Psoriasis Vulgaris

Objective:To observe the efficacy and its adverse effect.Methods:A total of 72 patients were treated with 311nm NB-UVB radiation only which was performed one time every other day,totally for 20 times.The efficacy was evaluated by psoriasis area and severity index(PASI).The pathobiology was evaluated and the Ki-67 and CD3 were detected by immunohistochemical techniques in lesional psoriatic epidermis,before,ten times and after the phototherapy in 20 patients.Results:①Clinical therapeutic effect:Following the phototherapy,the PASI in lesions with psoriasis were falling-off and obviously results were generated.When the tenth phototherapy, PASI in lesions was significantly lower than that before.The most of lesions were complete regression.The clinical efficiency is 91.7%.The efficiency of guttiform is better than that of enplaque.②The adverse reaction was few and gently.The 27.8%of patients appeared asteatosis cutis,pruritus.All of patients appeared hyperpigmentation.But after two or three months,the skins color recovered normal skim.All of patients accomplished the course of treatment.③Following the improvement of lesions,obviously alteration took place in histopathology.When the tenth phototherapy,histopathology showed:cuticularization excessive,parakeratosis is to lessen than that before;prickle cell layer thinningz;it's decreased that the phlegrnasia cells in dermal papillaes and reticular layeres.When the twentith phototherapy,histopathology is to get close that of normal tissue,④Ki-67 of lesions in psoriasis express highly than that of normal tissue before,which has extremely significance,which express mainly in Malpighian cells's cellular nucleus of stratum basale,stuatum spinosum and stratum granulosum.When the tenth phototherapy,The expression of Ki-67 in lesions was significantly lower than that before (p＜0.001).when the twentith phototherapy,which is to get close that of normal tissue.⑤CD3 of lesions in psoriasis express highly than that of normal tissue before,which has extremely significance,which express mainly in T leukomonocyte cells membrane of dermis superficial laye.When the tenth phototherapy,The expression of CD3 in lesions was significantly lower than that before(p＜0.001),When the twentith phototherapy,which is to get close that of normal tissue.Conclusions:Radiation of 311nm NB-UVB was safe and effective,which could be a routine method for psoriasis vulgaris.
