Background and ObjectiveArteriosclerosis (AS) is the main cause of diabetes mellitus (DM) complicating coronary disease, cerebrovascular disease and periphery blood vessel disease. Risk factors such as obese, hypertension and lipoprotein metabolism abnormality have higher incidence in T2DM than that in patient without DM. In addition, leptin as well as other cell factors have involved in the development of AS directly or indirectly.Leptin is a protein which is coded by obese gene (ob). Research results indicate that the mechanisms of serum leptin causing atherosclerosis in DM include of conducting endothelial cell multiplication, vascularization, oxidation stress, thrombus forms, the blood vessel smooth muscle cell migration and so on. In recent years, scientist have known that selectin family play the vital role in the the pathogenesis of diabetes with macrovascular diseases. The interaction between the white blood cell and the blood platelet, white blood cell and endothelial cell being mediated by P-selectin is one of initiating agent in the formation of arteriosclerosis. Serum ferritin (SF) is the main form of iron storage in body. Excess of SF may lead bad use of blood glucose by insulin resistence. The iron catalyze the generation of free radical, may participate in the occurrence of atherosclerosis by promoting lipid oxidation.This study is plan to find the relationships between T2DM with macrovascular complications and serum level of P- selectin, leptin and ferritin. The results may give a new way to the diagnosis, monitoring and evaluation of diabetic complications in the early stage.Materials and MethodsA case-control study was conducted. Forty subjects were divided into T2DM gioup and normal control group. The T2DM group was divided into macrovascular complication group (MVC) and non-complication group (Non-MVC). Healthy people were employed as Normal control group. ELISA technique was used to determine the level of serum P- selectin, leptin at the same time. The free insulin and ferritin were detected by using radioimmunoassay (RIA). By using correlation analysis and regression analysis, we analyze the correlation between all data and T2DM with macrovascular complication within and between groups.Results1. The relationship between leptin and T2DM with macrovascular complicationThe concentration of serum leptin increased in T2DM group compared with that in normal control group. The concentration of serum leptin increased in MVC compared with that in Non-MVC(p<0.05). In T2DM group. By using correlation analysis, we found that the concentration of serum leptin correlated positively with body mass index(BMI), low density lipoprotein cholesterol(LDL-C) and P-selectin, had no correlation with cholesterol(TC), triglyceride (TG), fast blood glucose (FBG) high density lipoprotein cholesterol(HDL-C) and ferritin, had negative correlation with free insulin (FINS). Multiple stepwise regression analysis revealed that the independent variable that was associated with leptin was P-selectin. Setting macrovascular complication dependent variable Y, and other indexes independent variables, leptin and LDL-C entered into regression equation by making logistic regression analysis.2. Setting macrovascular complication dependent variable Y, and other indexes independent variables, TG and LDL-C entered into regression equation by making logistic regression analysis.Conclusions1. The serum Leptin, P-selectin, and ferritin were significantly increased in T2DM group compared with that in control group;2. The serum Leptin, P-selectin, and ferritin were significantly increased in MVC group compared with that in Non-MVC group;3. The concentration of serum Leptin, p-selectin, and ferritin of patients with T2DM was related to obesity, blood glucose and lipid metabolic disorder;4. Leptin, P-selectin and ferritin were related to T2DM. Particularly,Leptin was close toT2DM with macrovascular complication. They might play an important role in the occurrence of atherosclerosis,and were important risk factors for diabetic macrovascular complication.
