Objective To investigate the changes of plasma levels of soluable P-selectin in type 2 diabetes mellitus patients with/without macrovascular complications ; to explore its role in the pathogenesis of the macrovascular complications of type 2 diabetes mellitus and its clinlical value.Methods The plasma level of soluble P-selectin was measured in 16 type 2 diabetes mellitus patients without vascular complications (simple diabetes mellitus group,SDM group), 25 type 2 diabetes mellitus patients with macrovascular complications(DM+MVC group) , 20 normal subjects (control group) by enzyme-linked immunosorbent assay. Plasma ox-LDL was determined by enzyme-linked immunosorbent assay simultaneously.Results (1) The plasma levels of sP-selectin and ox-LDL in SDM group were significantly higher than those in control group〔(90.13±19.99vs44.46±13.18)ng/ml, P<0.01; (70.61±11.44vs36.10±7.55) μg/dl,P<0.01,respectively〕. The plasma levels of sP-selectin and ox-LDL in DM+MVC group were significantly higher than those in control group 〔(108.17±16.44vs44.46±13.18) ng/ml,P<0.01;(108.39±21.40vs36.10±7.55)μg/dl,P<0.01,respectively〕. The plasma levels of sP-selectin and ox-LDL in DM+MVC group were significantly higher than those in SDM group〔(108.17±16.44vs90.13±19.99) ng/ml,P<0.05;(108.39±21.40vs70.61±11.44)μg/dl,P<0.01,respectively〕. (2) sP-selectin was positively correlated with BMI(r=0.416 ,p<0.01), SBP(r=0.459 ,p<0.01),DBP(r=0.449,p<0.01),TG(r=0.485, p<0.01),TC(r=0.312 , p<0.01),LDL-c (r=0.339 , p<0.05),HbA1c(r=0.732 ,p<0.01),FINS(r=0.305,p<0.05),FBG(r=0.549 ,p<0.01),HOMA-IR(r=0.502,p<0.01),respectively. (3)sP-selectin was positively correlated with ox-LDL(r=0.836,p<0.001). (4)Multiple stepwise regression analysis showed sP-selectin was influenced by ox-LDL ,HbA1c, BMI。Conclusion (1)sP-selectin may be a useful marker for the early diagnosing and monitoring the macrovascular complications of type 2 diabetes mellitus. (2)Anti-P-selectin antibody and its relevent research may be a new way to control the macrovascular complications of type 2 diabetes mellitus.
