ObjectiveThe expression of survivin and cyclinD1 was detected in osteosarcoma and osteochondroma.The osteosarcoma cells apoptotic level was also detected.Analyze the relationship between the expression of survivin and clinical and pathological factors and tumor cell apoptosis.Study the correlation of the expression of survivin and cyclinD1.Investigate their effect and interaction in the development and progression of oseteosarcoma.Methods(1)Case selection:All samples were from The second hospital,Shanxi Mendical University. Internalizing standard:①Osteosarcoma and osteochondroma tissue were from surgical operation resection;②Two experienced pathological doctors examined HE stain slice again to determine pathological diagnosis and Enneking surgical staging;③Primary tumor tissue paraffin block were saved;④All medical record data were reserved before and after operation.Exclusion standard:Clinical medical record data(name,sex,tumor site,follow-up visit data)could not be available.(2)Experimental group:46 cases osteosarcoma.By Enneking surgical staging,stageⅠ12 cases, stageⅡa 5 cases,stageⅡb 19 cases,stageⅢ10 cases.Tumor cell differentiation:High differentiation 9 cases,moderate differentiation 12 cases,poor differentiation 25 cases.Control group:38 cases osteochondroma.(3)The expression of survivin and cyclinD1 were detected by immunohistochemical ElivisionTM method in osteosarcoma and osteochondroma.(4)Cell apoptotic state was detected by TDT-mediated DNA in situ end labeling(TUNEL)method. Calculate apoptotic index(AI)and analyze the relationship between survivin and AI.Results(1)Only 2 cases in 38 osteochondroma tissues was detected slight expression of survivin protein. But in osteosarcoma,the rate of positive expression of survivin protein was 69.6%(32/46),which is significantly higher than that of osteochondroma(P<0.01).The positive rates of cyclinD1 in osteochondroma was 26.3%,and 65.2%in osteosarcoma.The difference between them was significant in statistic(P<0.01).(2)There was a relationship between survivin expression and Enneking surgical staging and tumor cell differentiation.The survivin expression was not correlated with sex and tumor site(P>0.05).The apoptotic index(AI)in positive expression of survivin was(6.23±2.27)%,and (9.71±2.34)%in negative expression of survivin.There was a statistical significance in values of AI between two groups.(3)There was a relationship between survivin and cyclinD1 expression.Of the positive cases for survivin protein,the positive rate for cyclinD1 was 87.5%(28/32).Of the negative cases for survivin protein,the positive rate for cyclinD1 was 14.3%(2/14).The expression of survivin was positively correlate with that of cyclinD1(r=0.635,P<0.01).Conclusion(1)Compared to the slight expression of survivin in osteochondroma,the expression of survivin in osteosarcoma presented specific up-regulation,which indicates that survivin promote the occurrence and development of osteosarcoma.The lower the osteosarcoma differentiated degree was and the later the osteosarcoma Enneking surgical staging was,the higher the expression of survivin was,which meant that the overexpression of survivin signified higher invasion and poor prognosis.(2)In osteosarcoma tissues,apoptotic cells mainly concentrate in the group of survivin negative expression.AI in survivin positive expression group was obviously higher than that in negative group.Survivin protein expression was negatively related to AI.That meant the expression of survivin in osteosarcoma resulted in cellular apoptosis mechanism disorder and enhanced capability of tumor cells to resit apoptosis.(3)The expressions of survivin was positively related with that of cyclinD1.It suggest that they may play congenerous roles during the onset and development of osteosarcoma.Combination of survivin and cyclinD1 detection may be helpful for the clinical and pathological diagnosis and provide valuable references for clinical treatment and prognosis evaluation.
