Objective: To study the expression of Syndecan-1,RAB5A,MMP-9,CD54 as well as relation to infiltration- -metastasis,tissue-typing etc, and significance in gastric carcinoma, to investigate the unite factor effect of Syndecan-1,RAB5A,MMP-9,CD54 for metastatic mechanism of gastric carcinoma and relation among each. Further understand metastasis of gastric carcinoma in different link, therefore, to look for a kind of more sensitive and reliable method for early period diagnosis and early period remedy in a molecule level of gastric carcinoma.Methods: S-P immunohistochemical technique is used to examine the expression and relation of Syndecan-1,RAB5A,MMP-9,CD54 in 46 cases of gastric carcinoma and the surrounding tissues of 3.0cm distance,10.0cm distance and 10 cases of normal mucosa(trauma or benignant ulcer to be verified by pathology). The level of the expression is analyzed combining with clinicopathological features of age and sex of the cases, the size of the tumor, infiltration depth, differentiation degree and lymph node metastasis. Results: The color of the immune positive products of Syndecan-1,RAB5A,MMP-9,CD54 is buffy and located in cytoplasm or plasmalemma. The rate of positive expression of Syndecan-1 is 15.22%(7/46), 28.26%(13/46),86.69%(40/46)and 100%(12/12)in gastric carcinoma, surrounding tissues of 3.0cm distance, 10.0cm distance and normal gastric mucosa respectively and the tendency of increase is obvious. There is no remarkable difference between idiopathic nidus and surrounding 3.0cm distance(P>0.05), but remarkable difference between idiopathic nidus and surrounding 10.0cm distance and normal mucosa(P<0.01). The rate of positive expression of RAB5A is 78.26%(36/46), 63.04%(29/46), 17.39%(8/46)and 0.00%(0/12)in gastric carcinoma, surrounding tissues of 3.0cm distance, 10.0cm distance and normal gastric mucosa respectively .The expression of RAB5A is increased in gastric carcinoma and has a significant difference with surrounding 10.0cm distance tissue and normal gastric mucosa(P<0.01). The rate of positive expression of MMP-9 is 82.61%(38/46), 65.22%(30/46), 10.87%(5/46)and 0.00%(0/12)in gastric carcinoma, surrounding tissues of 3.0cm distance, 10.0cm distance and normal gastric mucosa respectively. The expression of MMP-9 is increased in gastric carcinoma and has a significant differences with surrounding 10.0cm distance tissues and normal gastric mucosa(P<0.01). The rate of positive expression of CD54 is 67.39%(31/46), 54.35%(25/46), 8.07%(4/46)and 0.00%(0/12)in gastric carcinoma, surrounding tissues of 3.0cm distance, 10.0cm distance and normal gastric mucosa respectively. The expression of CD54 is increased in gastric carcinoma and has a significant difference with surrounding 10.0cm distance tissues and normal gastric mucosa (P<0.05)(table 1). The expression of Syndecan-1 is lower in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression is 5.26%(2/38)and 62.50%(5/8)respectively(P<0.01). The expression of RAB5A is higher in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression is 89.47%(34/38)and 25.00%(2/8)respectively (P<0.01). The expression of MMP-9 is higher in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression is 94.74%(36/38)and 25.00% (2/8) respectively(,P<0.01). The expression of CD54 is higher in moderate and poor differentiation degree than those in high differentiation degree and the rate of positive expression is 76.32%(29/38)and 25.00%(2/8)respectively(P<0.01). The expression of Syndecan-1 is higher in infiltrated range limited to muscle stratum than serosa stratum and the rate of positive expression is 41.67%(5/12)and 5.88%(2/34)respectively,(P<0.05). The expression of RAB5A is lower in infiltrated range limited to muscle stratum than serosa stratum and the rate of positive expression is 41.67%(5/12)and 91.18 %(31/34)respectively,(P<0.01). The expression of MMP-9 is lower in infiltrated range limited to muscle stratum than serosa stratum and the rate of positive expression is 58.33%(7/12)and 91.18 %(31/34)respectively,(P<0.05). The expression of CD54 is lower in infiltrated range limited to muscle stratum than serosa stratum and the rate of positive expression is 33.33 %(4/12)and 79.41 %(27/34)respectively(,P<0.05). The expression of Syndecan-1 is higher in gastric carcinoma without lymph node metastasis than with lymph node metastasis and the rate of positive expression is 42.86 %(6/14)and 3.13 %(1/32)respectively, (P<0.01). The expression of RAB5A is lower in gastric carcinoma without lymph node metastasis than with lymph node metastasis and the rate of positive expression is 42.86 %(6/14)and 93.76%(30/32)respectively, (P<0.01). The expression of MMP-9 is lower in gastric carcinoma without lymph node metastasis than with lymph node metastasis and the rate of positive expression is 57.14 %(8/14)and 93.76%(30/32)respectively, (P<0.05). The expression of CD54 is lower in gastric carcinoma without lymph node metastasis than with lymph node metastasis and the rate of positive expression is 35.71%(5/14)and 81.25%(26/32)respectively, (P<0.01). The rate of positive expression of Syndecan-1is 26.92%(7/26)in gastric carcinoma without distant place metastasis and no expression with distant place metastasis, ( P<0.01 ) . The expression of RAB5A is lower in gastric carcinoma without distant place metastasis than with it and the rate of positive expression is 65.38 %(17/26)and 95%(19/20)respectively, (P<0.05). The rate of positive expression of MMP-9 in gastric carcinoma without distant place metastasis is 69.23 %(18/26)and whole expression with i(tP<0.05). The expression of CD54 is lower in gastric carcinoma without distant place metastasis than with it and the rate of positive expression is 53.85 %(14/26)and 85.00 %(17/20)respectively, (P<0.05). The expression of Syndecan-1,RAB5A,MMP-9,CD54 had no significant differences in the age and sex of the cases, the size of the tumo(rtable 2, 3). The expression of Syndecan-1,RAB5A,MMP-9,CD54 is significant correlated among each(P<0.05)(table 4~6).Conclusion: Syndecan-1,RAB5A,MMP-9,CD54 may be closely correlated to the occurrence and the development of gastric carcinoma. The expression of RAB5A,MMP-9,and CD54 increase while Syndecan-1 decrease in gastric carcinoma. The expression of Syndecan-1,RAB5A,MMP-9,CD54 is significant correlated among each in gastric carcinoma. The decreased expression of Syndecan-1 as the first step of metastasis may make gastric cancer cell detachment while mutation of RAB5A gene cause gastric cancer cell to infinite multiply and has connection with adhesion and metastasis, MMP-9 destroy integrity of basic membrane and CD54 encourage gastric cancer cell to adhere with basic membrane, as a result, they bring about metastasis of gastric carcinoma together. Therefore, Syndecan-1,RAB5A,MMP-9 and CD54 can be checked by gene to enhance sensitivity of diagnosis and remedy early period.
