The development,invasion and metastasis of solid tumor depend on intratumoral angiogenesis which provides the necessary nutritive material.The process of intratumoral angiogenesis is regulated through the complex interplay between angiogenesis promotors and inhibitors. For instance, angiogenesis promotors include vascular endothelial growth factor(VEGF) and platlete-derived growth factor (PDGF) et al and angiogenesis inhibitors include endostatin et al.To research the mechanism of lymph node metastasis of oral malignent tumor by comparing the microvessel density (MVD) and the expressions of VEGF, PDGF and endostatin in carcinoma of tongue and its cervical part metastasis and the metastasis of lymph node developed after resection of the primary tumor.The expressions of CD34, VEGF, PDGF and endostatin are detected and analyzed by immunohistochemical technique in 40 cases of carcinoma of tongue with lymph node involvement and 20 cases of the metastases of lymph node developed after resection of the primary tumors.As a result,higher MVD are observed in the primary tumors and the metastases of lymph node developed after resection of the primary tumors than in the matastases which are resected homeochronously with the primary tumors (P<0.05), whlie the latter has stronger expression of endostatin than the other two groups (P<0.05).And the expressions of VEGF and PDGF are not significantly different among these three groups (P>0.05).There is a positive correlation among the expressions of VEGF, PDGF and MVD in the primary tumor (P<0.05),and there is not a correlation among the expressions of VEGF, PDGF and MVD in the matastases which are resected homeochronously with the primary tumors (P>0.05).There is a negtive correlation between the expression of endostatin and MVD in the matastases which are resected homeochronously with the primary tumors (P<0.05),and there is not a correlation between the expression of endostatin and MVD in the primary tumor (P>0.05)Based on these findings,we can draw conclusions as follows:1. There are higher MVD in the primary tumors than in their metastases which are resected homeochronously.The expression of endostatin in the primary tumors is stronger than in their metastases,but the expressions of VEGF and PDGF are not significantly different between these two groups.2. There are higher MVD in the metastases of lymph node developed after resection of the primary tumors than in the matastases which are resected homeochronously with the primary tumors.The expression of endostatin in the latter is stronger than in the former, but the expressions of VEGF and PDGF are not significantly different between these two groups.3. MVD and the expressions of VEGF, PDGF and endostatin are not significantly different between primary tumors and metastases of lymph node developed after resection of the primary tumors.4. There is a positive correlation among the expressions of VEGF, PDGF and MVD in the primary tumor, and there is not a correlation among the expressions of VEGF, PDGF and MVD in the matastases which are resected homeochronously with the primary tumors.There is a negtive correlation between the expression of endostatin and MVD in the matastases which are resected homeochronously with the primary tumors,and there is not a correlation between the expression of endostatin and MVD in the primary tumor.5. Primary tumor may inhibit the angiogenesis in its metastasis through secreting angiogenesis inhibitors such as endostatin.After the primary tumor is reseeted,the metastasis can develop continuously for the lack of angiogenesis inhibitors.
