Research On Discriminant Analysis For Rheumatoid Arthritis Of TCM Pattern Identification

Objective:According to the discriminant analysis of the database which established on the grade score of rheumatoid arthritis(RA)related symptoms and signs,we set up the discriminant formulas of damp-heat impediment pattern,dual vacuity of liver and kidney pattern and cold-heat complex pattern,and hoped to provide a theoretical basis for Traditoonal Chinese Medicine(TCM)on the diagnosis and treatment of RA.Methods:Collect the information of RA patients which meet the diagnostic criteria Rheumatism Association of the United States in 1987,and diagnosed them the three pattern types(according to " Chinese medicine clinical research guidelines for new drugs" compiled by Zheng Xiao Yu in May,2002 and " Practical Rheumatology of TCM"compiled by Jiao Shude and Lu Zhengzhi in Febuary,1998):damp-heat impediment pattern,dual vacuity of liver and kidney pattern and cold-heat complex pattern.Use the EPIdate3.1 statistics software package to carry inputting management on the date and analyse by SPSS11.0 software package,set the inspection level asα=0.05.The analysis chooses gradually forward the biggest likelihood estimate module gradually with Analyze-Classify-Discriminant.Set every symptoms and signs in each syndrome as a variable to do discriminant analysis,and then select the significant contribution ones to establish discriminant function,after that use the way of one by one back to test the coincidence rate.Results:Patients include 436 cases,the damp-heat impediment pattern 129 cases, accounting for 29.6 percent,the dual vacuity of liver and kidney pattern 209 case, accounting for 47.9 percent,the cold-heat complex pattern 98 cases,accounting for 22.5 percent.88 cases are male,accounted for 20.2 percent,and female 348 cases,which accounted for 79.8 percent,the ratio of male and female is 1:4.Accoring to the different classification variables group on TCM syndromes,we screened 13 variables gradually: joint tenderness,fever,yellow urine,local touching fever with consiciously chills, consciously fever without local touching fever,light red tongue,yellow thick coating, yellow greasy coating,yellow-white coating,yellowish coating,deep-thin-wiry pulse, slip-frequent pulse,slip-wiry pulse,which all contributed significantly in the TCM distinction.We set up Linear Discriminant Function about RA on TCM pattern types:The damp-heat impediment pattern=—6.886 + 1.914 joint tenderness + 0.924 fever + 2.222 yellow urine +0.704 local touching fever with consiciously chills + 0.829 consciously fever without local touching fcver+1.210 light red tongue+2.955 yellow thick coating+ 3.479 yellow greasy coating+ 1.315 yellow-white coating+ 3.441 yellowish coating+ 9.087 deep-thin-wiry pulse+ 7.128 slip-frequent pulse + 7.729 slip-wiry pulse.The dual vacuity of liver and kidney pattern=－6.886+1.914 joint tenderness+ 0.924 fever+2.222 yellow urine+0.704 local touching fever with consiciously chills+ 0.829 consciously fever without local touching fever+1.210 light red tongue+2.955 yellow thick coating+3.479 yellow greasy coating+1.315 yellow-white coating+3.441 yellowish coating + 9.087 deep-thin-wiry pulse + 7.128 slip-fiequent pulse + 7.729 slip-wiry pulse.The cold-heat complex pattcm=－6.886+1.914 joint tenderness+0.924 fever+ 2.222 yellow urine + 0.704 local touching fever with consiciously chills + 0.829 consciously fever without local touching fever+1.210 light red tongue +2.955 yellow thick coating+ 3.479 yellow greasy coating+ 1.315 yellow-white coating+ 3.441 yellowish coating + 9.087 deep-thin-wiry pulse + 7.128 slip-frequent pulse + 7.729 slip-wiry pulse.The above three discriminant functions are very in accordiance with the classification of the original data as the rote 86.8 percent,81.8 percent,91.8 percent separately。Conclusion:Based on the statistical probability,we quantified the symptoms and signs of RA and make them as the important base of diagnosis & diffrential diagnosis after probability computing.It will play an active function on improving the TCM criteria of diagnosis,diffrcntial diagnosis,Standardized and Quantitative Syndrome Research.It improved that the discriminant functions have a great discrimination and can give a high value on RA of TCM syndromes.
