The activation of leucocyte and adhereing to endothelial cell is criticality factor to ischemic stroke,also is activated platelet. L-selectin(CD62L)and P-selectin(CD62P)play an important role in thrombosis.In acute stage of stroke,as an important adhesion molecule of cycling neutrophilic granulocyte to express,CD62L mediates rolling of neutrophilic granulocyte and it is precondition of neutrophilic granulocyte fast adhere to endothelial cell.CD62P belongs to selectin family of adhesion molecule,and it is a very little expressed on membrane of silent platelet,when platelets are activated,the specific membrane protein P selectin on the membrane of a granules in the cytoplasm of platelet cells is translocated with the degranulation of the activated platelets and is fused with the plasma membrane of platelets,which is expressed on the plasma membrane of platelets,becoming the specific molecular marker in the recognition of activated platelets,and mediating the adhesion and association of the platelets with the granular and endothelial cells.The physiology mechanism of progressive ischemic stroke isn't very clear,We observed the level of L-selectin and P-selectin expression in the peripheral blood of the progressive cerebral infarction patients in order to verificate whether it could be an objective index of progressive cerebral infarction or not.ObjectiveTo observe expression and clinical significane of L-selectin and P-selectin in the peripheral blood of the progressive cerebral infarction patients.Method1.Subject(1)progressive ischemic stroke group:20 patients were males, and 12 patients were females.The rang of age was from 39 to 83,with the average age was 64.56±12.45 years.(2)Complete stroke group:56 patients were males,and 52 patients were females.The rang of age was from 42 to 85,with the average age was 64.06±10.30 years.(3)Control group:there were 40 health examination people.They were also examined by head CT or MRI whose results showed that they weren't cerebral infarction patients.22 cases were males,and 18 cases were females.The rang of age was from 44 to 72,and with the average age 63.28±8.11 year old.The subjects with diabetes,heart disease,lung disease,liver disease, kidney disease,hematonosis,infection or tissue damage were excluded. The subjects who had taken medicine such as aspirin,ticlopidine,agacore or heparin and so on in recent two weeks were excluded.2.Method(1)CD62L detection:Fluorescence antibodies of CD62L PE(20μl) and whole blood(100μl)were put into the experiment test tubes,then hemolysin was put into it and was centrifuged,after it,PBS was put into the tubes and centrifuged again.Eventually,we put paraform into test tubes and detected them by flow cytometry.(2)CD62P detection:Fluorescence antibodies of CD61 PerCP,CD62P PE(each 20μl)and whole blood(5μl)were put into the experiment test tubes,and fluorescence antibodies of CD61 PerCP,IgG1 PE(each 20μl),RGDS(10μl)and whole blood(5μl)were put into the contrasting test tubes.Let them react under some conditions so that platelets would be dyed by fluorescence antibodies.Then we detected them by flow cytometry.(3)The patients who were selected were draw blood to check when they were admit to hospital immediately and were recheck at 7th day and 14th day.Results(1)Compared with the complete stroke group and control group, positive rates of CD62L were lower and positive rates of CD62P were higher within 2 day and at 7th day in patients with progressive ischemic stroke group(P<0.05)(2)With the progressing of the course,positive rates of CD62L were higher gradually,but positive rates of CD62P were lower gradually in patients both progressive and complete stroke group.On 14th day, Compared with control group,positive rates of CD62L were lower in patients with progressive ischemic stroke group(P< 0.05),but positive rates of CD62P without obvious difference(P> 0.05)(3)There was a statistically negative correlation between CD62L expressions and CD62P expressions(r=-0.602,P<0.01)within 2 day, while there was not obviously correlation between CD62L expressions and CD62P expressions at 14th day.ConclusionThe investigation indicated the positive rates of CD62L are significantly lower and positive rates of CD62P are significantly higher in patients of progressive ischemic stroke at acute stage.In 14th day,the subacute stage,the expression rates of CD62L and CD62P were not obviously difference compared with control group.The level of CD62L and CD62P could be an index to observe the occurring and developing of progressive cerebral infarction.And it is helping to evaluate prognosis and direct to therapy.
