Objective To observe the relationship between the serum levels of Ig-G antibodies of helicobacter pylori(Hp)of the patients with unstable angina pectoris(UAP)and abnormal glucose metabolism.Methods One hundred and fourteen patients with UAP of Tianjin Medical University General Hospital,recruited consecutively from February 2006 to December 2007,as UAP group were enrolled,(57 men,57 women)age 36~81(61.32±9.00)years.The patients with UAP were divided into 2 groups according to results of oral glucose tolerance tests(OGTT):29 patients with normal glucose tolerance(NGT)(13 men,16 women),age 37~76(58.83±8.86) years.85 patients with abnormal glucose metabolism[include impaired glucose tolerance(IGT)and type 2 diabetes mellitus(DM)](44men,41women),age 36~81(62.18±8.94)years.The patients with UAP were divided into 2 groups according to insulin resistance index:82 patients with non-insulin resistance(45men,37women), age 36~79(61.37±8.85)years.32 patients with insulin resistance(12men,20women), age 37~81(61.22±9.53)years.There were 38 patients with stable angina pectoris(SAP)(23men,15women),age 43~81(62.45±10.00)years in control group recruited according to the diagnostic criteria during same period.If patients with other conditons that can affect the serum concentration of the Ig-G antibodies to Hp(Hp-IgG),hsCRP and glucose metabolism were excluded.All blood samples were extracted in the morning.The serum was centrifugal separated after standing,kept in -80℃freezer,and sent to determine together.The concentration of Hp-IgG were determined by the enzyme linked immunosorbent assay(ELISA).The concentration of the hsCRP were determined by the immunoturbidimetry while the concentration of TC,TG,HDL-C,and LDL-C were determined.The OGTT were performed by chemoluminescence technique.The degree of insulin resistance was assessed using insulin resistance index by homeostasis model assessment(HOMA IR).Coronary angiography was performed in all patients to assess the degree of coronary artery stenoses.Results The serum levels of Hp-IgG,hsCRP,fasting plasma glucose(FPG),HOMA IR and Gensini's scores are higher in UAP group than in the control group. Multivariable logistic regression analysis show that the higher levels of serum Hp-IgG(OR=5.130,95%CI 1.910~13.782,P=0.001),hsCRP(OR=3.577,95%CI 1.325~9.654,P=0.012),FPG(OR=6.196,95%CI 1.211~31.424,P=0.028)are significantly associated with UAP.The levels of FPG,HbA1c and HOMA IR and the degree of coronary artery stenoses are higher in the UAP patients with abnormal glucose metabolism than those in the SAP patients with NGT and the UAP patients with NGT(P<0.05).The plasma levels of TG(P<0.05),FPG,FINS,HbA1c and hsCRP are higher in the patients with insulin resistance than those in the patients with non-insulin resistance(P<0.01).The serum levels of Hp-IgG and Gensini's scores are no significant difference between the UAP patients with insulin resistance and with non-insulin resistance(P>0.05).The serum levels of Hp-IgG is no significant difference between the UAP patients with abnormal glucose metabolism,the UAP patients with NGT and the SAP patients with NGT(P>0.05).No significant correlations were found between the serum levels of Hp-IgG and the levels of FPG (Spearman correlations:r=0.047,P=0.622),hsCRP(r=-0.148,P=0.120)in UAP patients.Conclusion The serum levels of Hp-IgG,hsCRP,fasting plasma glucose(FPG),HOMA IR and Gensini's scores are higher in UAP group than in the SAP group.Multivariable logistic regression analysis show that the higher levels of Hp-IgG,hsCRP and FPG are significantly associated with UAP.Hp infection may participate in development of UAP.Abnormal glucose metabolism not only was associated with UAP,but also aggravated the degree of coronary artery stenoses in the UAP patients.Our results suggest that Hp infection may participate in development of UAP,which role independent of abnormal glucose metabolism.
