Objective:To observe the influence of cluster acupuncture on scalp points (CASP)on cerebral edema and inflammatory reaction following cerebral ischemic reperfusion(CIR)injury in rats,and try to reveal the cerebral protection effect and possible mechanism of CASP.Methods:Rat model of focal cerebral ischemia reperfusion was established by improved middle cerebral artery occlusion(MCAO)method.Male Wistar rats were divided randomly into three groups:sham-operated control group, cerebral ischemia-reperfusion model group and CASP group,and each group was separated into 6 hours,24 hours,48 hours,72 hours,120 hours after reperfusion as subgroup for investigation.(1)The changes of brain water content were measured by the dry-wet method;(2)HE staining was used to observe the changes of pathomorphology;(3)The expression of AQP4 in CIR rats was measured by immunohistochemical method respectively and the influence of CASP on it was observed;(4)The concentration of TNF-αin plasma was measured by enzyme-linked immunosorbent assay method and the influence of CASP on it was observed.Results:(1)Compared with sham-operated control group,the brain water content increased markedly in model group and CASP group at every time points and there was significant difference between each group(P<0.05);The brain water contention in CASP group was significantly lower than that of model group at the time points of 72h,120h after reperfusion(P<0.05,P<0.01), but no difference at the time points of 6h,24h,48h after reperfusion(P>0.05); (2)HE staining results:In sham-operated control group,the structure of cerebral tissue were integrity,nerve cells,glial cells and capillary were eumorphism,and focal necrosis and inflammatory cell infiltration were not discovered at every time points.In model group,there was evident dropsy in brain tissue,neuron and gliocyte swelling,degeneration and necrosis and inflammatory cell infiltration which were obvious at the time points of 48h and 72h after reperfusion.Compared with model group,at the time points of 72h,120h after reperfusion,tumefaction of nerve cells,edema of stroma,and inflammatory cell infiltration have been decreased,and the amount of neurons and new capillaries have been increased in CASP group;(3) Immunohistochemistry results:Compared with sham-operated control group, the expression of AQP4 decreased at the time points of 6h~72h in model group and the time points of 6h~120h in CASP group and there was significant difference between each group(P<0.05);Compared with model group,the expression of AQP4 has been significantly decreased at the time points of 24h~120h(P<0.05,P<0.01).(4)ELISA results:Compared with sham-operated control group,the contents of TNF-αin plasma obviously increased at the time points of 6h~72h in model group and CASP group,and there was significant difference between each group;Compared with model group,the contents of TNF-αin CASP group have been significantly decreased at the time points of 24~72h after reperfusion(P<0.05,P<0.01).Conclusion:(1)CASP method can decrease the brain water content following CIR to relieve cerebral edema.(2)CASP method can decrease the tumefaction of nerve cells,the edema of stroma and the degree of inflammatory reaction,and promoted the repair of damaged neurons and the hyperplasia of capillaries and neuroglia cells and embody its plasticity of neuroanatomy.(3)CASP method can regulate AQP4 expression after CIR to relieve cerebral edema.(4)CASP method can decrease the level of TNF-αin plasma to relieve inflammatory reaction after CIR.
