Research Background:Because of the high incidence which increases with age,recurrence rate,and disability rate,ischemic stroke has now become the first disease causing death in China.To search the mechanism of ischemic stroke is the key of curing the disease.Neurovascular unit is constructed with vascular endothelial cells,pericyte,glial cells,and neurons and has complicated structures and functions.The vascular endothelial cells and astrocyte construct the blood brain barrier,which regulates material exchange between brain and peripheral blood as a tangible protection layer.Astrocyte can also secrete a variety of neurotrophic factors to improve the growth and differentiation of vascular endothelial cells and neurons.Glial cells function like macrophage as the immune cells in brain.Glial cells are usually in quiescent condition.After brain was injured with ischemic stroke,glial cells become active.They can regulate the permeability of blood brain barrier,inflammatory,autophagy,etc.As for now,there are still many questions of the different parts of neurovascular unit's action after ischemic stroke.But with the development of the science,combine treatments on multi-targets becomes new hope of curing ischemic stoke.Acupunctre is a characteristic therapy of Traditional Chinese Medicine(TCIM).Doctors summed up their own unique therapies with years of clinical practice.Professor Yu Zhishun,from Hei Longjiang province,combines the law of human meridian in TCM and the functional areas of the brain and the distribution of nerves based on modern anatomy and creates the cluster needling of scalp acupuncture therapy which is effective in clinical practice.Whether the effectiveness of the therapy is by taking part in rebuilding the dynamic balance of neurovascular unit,how does all these things works,and whether there is special target are the key points of research of cluster needling of scalp acupuncture therapy and a new direction for studying the mechanism of ischemic stroke.In the pathogenesis of ischemic stroke,immune actions are surely a key point.CD11b is an important substance that mediates the adhesion of macrophages to target cells and regulates the phagocytosis process and its only source in blood brain barrier is microglia.So CD11b is the marker protein of microglia and the evaluation index of phagocytosis of microglia.Under normal condition,the expression level of CD11b is very low.If microglia was activated,the expression of CD11b will soon increase significantly.This study researches the mechanism of cluster needling of scalp acupuncture therapy by obtaining the change of nerve function scores and CD11b expression of MCAO rats.Objective:To observe the effect of cluster needling of scalp acupuncture on MCAO rats through nerve function score and CD11b expression.Method:54 male SD rats in total were randomly divided into model group,acupuncture group and sham operation group.The rats in each group were randomly divided into three sets that would be drew material at the 2nd,7th and 14th day and were normally bred.The model group and the acupuncture group rats were made to be the MCAO model by the suture method.The sham operation group had only the arterial detachment operation.Model group and sham operation group accepted no treatment,while acupuncture group accepted cluster needling treatment.All rats were scored 4 hours after operation and before had been drew material.After perfusion,penumbra of MCAO rats were stored at-80 °C.The contents of CD11b were detected by western blot.Results:1.Neurological scoresComparing in the same group,the 7th day set and the 14th day set of acupuncture group were lower compared with the 1St day set(P<0.05),scores of the 1st,7th,14th day sets in model group were decreased,but there was no statistical significance.Comparing between the different groups,score's decrease of both the 7st day set and the 14th day set in the acupuncture group was larger than the model group's(P?0.05).2.Detection of CD1 1bThe expression of CDllb of sham operation group had no significant change.The expression of model group rose to the summit at the 1st day and fall at the 7th day.These changes were both statistically significant(P<0.05).The expression of CDllb of acupuncture group had only slightly change in three different time point(P>0.05),and the pression arrived at the summit at 7th day.Comparing among different groups,the expression of model and acupuncture group both significantly increased in the 1st day(P<0.05);at the 7th day,the pression of model group was slightly higher than that of sham operation group(P>0.05),and the pression of acupuncture group was obviously higher than that of sham operation group and model group(P<0.05);at the 14th day,the expression of acupuncture group was slightly higher than that of model group(P>0.05)while the expression of model group was slightly higher than that of sham operation group(P>0.05).Conclusion:1.The cluster needling therapy can decrease the score of MCAO rats and improve the nerve function.2.The cluster needling therapy can inhibit the expression of CD1 lb in the first day after CIS,and maintain the higher expression level of CD11b in 14 days.
