Objective: IgA nephropathy(IgAN) is a frequent primary glomerulopathy disease of our country, about accounting for 26%-34% in primary glomerulopathy nephropathy. It trends to incease in recent years. We discorver that prognosis is not optimistic with the increasing development of the study and long time follow-up. 20%of patients make advancement terminal stage kidney failure after 10 years. 30%of patients make advancement terminal stage kidney failure after 30 years. Another 30% of patients show different degree renal function to descend. Early diagnose and treatment undoubtedly is a important topic in nephrology. At present, treatment of western medicine for it merely stays symptomatic treatment stage.The effect is not ideal. Chinese medical gets a satisfactory therapeutic effect by binding modern patho-study and individual condition of patients on controling hematuria and preserving renal function . Professor Zhaoyuyong by clinical observation for many years applicates collateral disease theory to described Chinese medical pathomechanism of IgAN. He thinks pathogenetic condition of IgAN is difficult to recover and course of disease is long. Long disease must be deficiency. The prolonged disease affects collaterals. Kidney collateral stasis is basic pathogenesis of IgAN. He uses the method of replenishing Qi and disperse blood stasis and determination of treatment based in pathogenesis obtained through differentiation of symptoims and signs to make use of replenishing Qi medicine and worm medicine which can promot blood flow and disperse blood stasis and dredge collateral to heal IgA. He creates shenluotong( including astragali,black snake,cicadaslough,Hoelen,danshen and so on) which is predominant on clinic therapeutic effect. Empirical study result also showed disperse blood stasis and dredge collateral might delay nephric tubule interstitial fibrosis and glomerular sclerosis, redusing vasoactive substance as AGT 2,angiotensin converting to be secreted and ECM to be deposited,inhibiting transforming growth factor over expression , improving renal function. We treat IgAN with the method of replenishing Qi and disperse blood stasis and SLT. We will observe clinic curative effect, supplying valid method and research way for Chinese medicine treatment of IgAN.Methods:we choosed 95 patients of IgAN who were in the stages of hematuria in Hebei Medical University affiliated traditional Chinese medical hospital and traditional Chinese medical college out-patient department . We divided them into 2 groups randomly and comparable. 52 cases were in the treated group, 43 in the controled group. There were not significant difference in two groups. The treated group was treated by basic treatment and SLT. The controled group was treated Panshengding. The treatment course is 3 months. We reviewed clinic symptom,24 hours urine protein,urine red blood cell count and abnormity rate,serum immunoglobin,serumβ2-MG,urineβ2-MG,urine IgG,Alb.Results :We reserved 90 patients at fast,50cases in the treated group,40 cases in the controlled group.1 Total effective rate in the treated group was 82% and 57.5% in the controled group. There were significant difference in both groups ( P <0.05).2 In the treated group clinic symptom integral had significant difference after treatment (P<0.01). Clinic symptom integral had significant difference betwern the two groups( P<0.05) .3 In the theated group 24 hours urine protein,urine red blood cell count and abnormity rate had significant difference after treatment (P<0.01). Every index had significant difference betwern the two groups( P<0.05,P<0.01).4 Serumβ2-Mg,urineβ2-MG,urineIgG,Alb had significant difference after treatment in the treated group( P<0.05, P<0.01). There were significant difference in both groups (P<0.05). 5Serum IgA had significant difference after treatment in the treated group (P<0.05). IgA were significant difference in both groups(P<0.05).Concludsion: 1 The method of replenishing Qi and disperse blood stasis and SLT can improve clinic symptom, and reduce 24 hours urine protein,urine red blood cell count,improve immune capability.2 Under the guidance of collateral disease theory, there are remarkable curative effect by the method of replenishing Qi and disperse blood stasis to treat patients of IgAN. It can delay progression of disease. There are no side effect .It is a ideal method.
