objective:To access the security,clinical effect of percutaneous nucleoplasty on nerve root cervical vertebra and provide a mentality and a method for minimal invasive treating nerve root cervical vertebra on clinical.method:1.experimental histology observation:we select 2 hybrid watchdogs for animal experimentation,under 40-70temperatures,do percutaneous nucleoplasty on dog's neck intervertebral disc and observe the histology change,of the cervical vertebra nucleus pulposus,the cartilage and the intervertebral organization.2.Clinical curative effect observation:Strictly choose cases of illness according to the standard of the nerve root cervical vertebra sickness and use the lamination stochastic method to assign the cases to the treatment group or the control group,the specialist carries on comparison grading about the nerve root cervical vertebra sickness host sickness referencing to NRS11 and VAS which formulates from USA National Institute of Health clinical around the treatment.Use the integration to compare two group of overall curative effects and the main symptom difference;carry on 1~2 years questionnaire to the patient to fill out a form about the revisit,and uses Sampath about curative effect evaluation standard on nerve root cervical vertebra to Evaluate treatment effect.result:1.The histology observation:Drills the ablation using the low temperature plasma ablation technique,we can only find a diameter not to the 2mm passageway in the nucleus pulposus,obviously the Coblation ablation passageway has the bright and clean heat to congeal the edge,around the passageway organization have the hot shrinkage,but the cell still maintained activeness,the approaching ablation region textile fiber link and the lamina terminalis does not have the damage,also the approaching ablation area spinal cord and the nerve root were normal,the nerve cell and the collagen organization did not have necrosis or the damage.2.Clinical curative effect observation:Based on Sampath curative effect evaluation standards and VAS,make a follow-up visit form 3~14 months,the average 10 months,after statistics analysis,the comparison difference has statistics significanceafter about before or after of curation(P<0.001),and the last revisit subjective degree of satisfaction fine rate is 78.0%.The treatment group and the control group reveal rate respectively are 76.47%and 52.38%,the total effectiveness respectively is 94.12%and 73.81%;the near intermediate stage curative effect of The treatment group is affirmed,the forward curative effect depending on revisit, the treatment group and the control group have the significance difference in the recurrence rate aspect,two groups all have not seen the obvious complication.conclusion:Based on Strictly choosing cases of illness,nucleoplasty is an effective,minimally invasive and safe procedure for the patients with nerve root cervical vertebra,curative effect of the near intermediate stage is satisfied,the forward curative effect depending on revisit; it is the effective method that another kind of treating nerve root cervical vertebra besides traditional method and surgery,it was worth the clinical promoting and applicating in the clinic.
