Objective:1, To investigate the relation between expression of c-FLIP and RECK in laryngeal carcinoma and its clinical characteristics; To approach the changes in characteristics of c-FLIP and RECK in laryngeal cancer and adjacent tissues of cancer; To explore the biological action of laryngeal tumor and the range of safe surgical margin.2, To research the characteristic of RECK in supraglottic laryngeal carcinoma; To explore distance of submucosal invasion about supraglottic laryngeal carcinoma and the safe range of submucosal surgical margin.Methods:1,Thirty-eight laryngeal squamous cell carcinoma (LSCC)specimens and different regions adjacent carcinoma (2 mm,5 mm,10 mm) were examined for the expression level of c-FLIP and RECK protein by IHC method (PV9000) and RT-PCR which was employed to measure the expression of c-FLIP and RECK mRNA. Thirty-eight Normal mucosa(15mm) were used for controls.2, Twenty-four supraglottic laryngeal squamous cell carcinoma and different distances of submucosal surgical margin(2mm,5mm,10 mm,15mm) were examined for the expression level of RECK protein by IHC. Using image analysis techniques to calculate the average optical density of protein expression in the value of quantitative analysis. Analysis of variance between the two groups using ANOVA and SNK test.Results:1,The expression of the two cytokines was significantly associated with clinical staging,pathological grading and cervical lymph node metastasis.2,The results also showed that the expression of c-FLIP decreased as tissue progressed in sequence from carcinoma tissues to 2mm,5mm,10mm,15mm distant mucosa adjacent carcinoma,and the expression of RECK increased. The statistical difference between carcinoma tissues and edge group (5mm,10 mm,15 mm) was significant (P< 0.05) respectively,but it was not significant between carcinoma tissues and tissues near the tumor (2mm).3, The expression of RECK protein in supraglottic laryngeal carcinoma was significantly associated with clinical staging and pathological grading.4,The expression of RECK protein in supraglottic carcinoma and different distances of submucosal surgical margin show increasing trend from carcinoma tissues to 2mm,5mm,10mm,15mm distant mucosa adjacent carcinoma. Determination of average optical density value derived cancer tissue, 2mm,5mm at the organization was no significant difference among the expression (P= 0.088),10mm and 15mm at tissue expression is also no difference (P= 0.251), but the first three and the last two have significant differences (P<0.01).Conclusion:1, The expression of the two cytokines in the tissue of larynx squamous carcinoma is significantly correlated with clinicopathological indexes of the tumor. 2, The two cytokines might participate in occurrence and development of larynx squamous carcinoma. c-FLIP and RECK may act as a reference index in judging the biological speciality of LSCC.3, It is appropriate to regard 5mm away from tumors as resection margins of mucosal for surgical treatment of laryngeal carcinoma. 4, The submucosal surgical margin of supraglottic laryngeal carcinoma should be achieved between 0.5cm and lcm. Postoperative follow-up remains to be further confirmed.
