The Relevance Experimental Research Of Huo-xue-bu-shen Decoction On The Therapy Of Renal Anemia

Chronic renal insufficiency (CRI), is also known as chronic renal failure (CRF)。It is a clinical syndrome that because of renal units seriously suffering from various reasons and glomerular filtration rate sustaining declining, which finally cause the kidney diffused pathological changes and disorder of the organism excretion and the regulating function. Renal Anemia is one of various complications during the process of chronic renal insufficiency. Many reasons cause the insufficiency of renal Epo or toxic matters in uremia plasm disturb the erythrocyte's generation and metabolism, cause inflammation response, innutrition and bleeding tendency,then finally the symptom of anemia. It severely affect the patient's duration and life quality.Modern medicine considers that its Pathogenesis relates to the following factors: insufficient EPO and using failure; red blood cell growth inhibiting factor ;the insufficiency of hematopoietic raw material;the toxic substance in the blood;magnanimous free radicals;too much PTH;hypersplenism;the decreasing of platelet numbers and abnormal function.RA belongs to scopes such as blood deficiency, dropsy, Shenlao,Xulao and so on, in Traditional Chinese Medicine. The key pathogenesis of RA can reduce to two points. One is ZhengXuXieShi, the other XuShi mixture.It is a syndrome at the end of the kidney disease containing Pishen insufficiency,Qixue insufficiency,Yuxuezhuo du retention, Mailuo blocking.During the recent years,there are more and more effective fangji relating to RA ,the treatment is becoming more rich, Traditional Chinese Medicine displays its superiority in many fields such as alleviating symptom; enchancing immune response capacity; improving the patient's life quality;postponing condition progress and reducing the mortality rate. Therefore,enhancing the reseachment of RA in the Traditional Chinese Medicine field is very important.This experiment with the instruction of my teacher, According to the Zhengti and Bianzheng theory of Traditional Chinese Medicine,undering the clinical reseaching of modern Chinese medicine,using Jianpiyishen,Huoxuehuayu and Jieduxiezhuo methods, chosed Rhubarb,Angelica,Astragalus,Salvia miltiorrhiza, Xianlingpi, Leonurus and herba taraxaci mixing a kind of decoction,for they can protect the kidney function,improve the anemia condition.In the meanwhile, Observed the changes of serum urea nitrogen, creatinine, red-cell count, hemoglobin, erythrocrit and haemopoietin of male rats.In the experiment,We chose 12 Wistar male rats as blank compared group from 60 rats. And that, the left rats were fed with 2% adenine by the way of 300mg/kg/d for renal anemia model,one time each day,continued thirty days.The successfully moding rats were divided into three groups with 12 rats in each:model group; traditional chinese medicine group(treatment group) ; and Ligustrazine group. Traditional chinese medicine group were clysised with 2ml mixtrue each day,Ligustrazine group were clysised with 2ml Ligustrazine starch suspension each day. The rest groups were clysised with equal nomal saline(2ml) each day. This process lasts for six weeks.After that, we took serum urea nitrogen, creatinine, red-cell count, hemoglobin, erythrocrit, haemopoietin as the detected indicators and compared the changes of each group.The experimental results showed that: 1.Huo-Xue-Bu-Shen Decoction can improve the renal function of renal anemia rats. The indicators of treatment group such as BUN and Scr had small difference comparing with normal group. This data was lower than that both of ligustrazine group and model group, which had the significant difference(P<0.05 or 0.01).The generally condition of treatment group such as body weight , state of mind and response is all better than ligustrazine group.2.Huo-Xue-Bu-Shen Decoction can improve the anemia status of the renal anemia rats. The indicators of treatment group such as red-cell count, hemoglobin were all obviously higher than that of model group (P<0.01). Compared with ligustrazine group, the indicators were also higher evidently(P<0.05). Erythrocrit, haemopoietin was lower than the normal group,but had significant difference(P<0.05) comparing with model group and before the treatment.Through the experimental data,we can see that the Huo-Xue-Bu-Shen Decoction can improve the renal function of RA rats,increase the level of red-cell count, hemoglobin, erythrocrit, haemopoietin, meliorate anemia status of chronic renal insufficiency. Rhubarb and Angelica in Huo-Xue-Bu-Shen Decoction can restrain the exudation and multiplication of glomerulal mesangial cell. Meanwhile, it can also reduce the synthesis of extracellular matrix, stop the proliferation of renal interstitial fibrocyte, accelerate protein synthesis ,accommodate the function of immune system, clear free radicals, postpone the procedures of glomerulos- clerosis and renal interstitial fibrosis.Astragalus,Rhubarb and Salvia miltiorrhiza in the prescription can reduce intestinal absorption of amino acid,inhibit the decomposition of protien in vivo,accelerate protein synthesis,reduce urea nitrogen synthesis,improve the elimination of each type of toxic matters affecting the kidney such as urea nitrogen and creatinine to protect renal function.Salvia miltiorrhiza, Angelica, Xianlingpi, Leonurus and herba taraxaci in the prescription can restrain platelet aggregation and activity ,reinforce fiber protein solubility,restrain the formation of microthrombus,improve microcir- culation , expand renal artery, decrease peripheral vascular resistance, increase renal plasma flow, improve Lipid Metabolism, enhance the activity of SOD and GSH-Px ,play the role of anti-oxidation and clear free radicals,enchance the immune capacity ,improve renal anemia hemorheology.Astragalus , Rhubarb, Angelica and Xianlingpi can level up colony stimulating factors as well as stimulating immune system, enchance immune response capacity. Moreover, those elements can boost formation rate of marrow DNA, can prompt multiplication of hemopoietic stem cell, stimulate the secreting of EPO. This prescription also have benefits for blood cells'formation, develo- pment,maturation so as to improve bone marrow micro-environment , clear free radicals ,protect Red blood cell membrane ,accommodate the function of hematopoietic system.In summary, Huo-Xue-Bu-Shen Decoction can restrain the exudation and multiplication of glomerulal mesangial cell, reduce the synthesis of extracellular matrix, postpone the procedures of glomerulosclerosis and renal interstitial fibrosis, inhibit the decomposition of protien in vivo,accelerate protein synthesis, improve the elimination of each type of toxic matters affecting the kidney, improve renal anemia hemorheology,clear free radicals,improve Lipid Metabolism,reduce the pathology harm of kidney, protect renal function, improve bone marrow micro-environment, accommodate the function of immune system, accommodate the function of hematopoietic system, meliorate anemia status.
