Background:There are a large group of patients with liver disease(LC), almost all the diseases ,which can induce slow liver injury, can cause liver fibrosis, and then may develop into LC. Early detection of LC and its correct diagnosis and the severity classification directly affects the treatment and prognosis of patients. In recent years, non-invasive imaging and serum fibrosis markers examnation has attracted the attention of scholars. Both of them have very important values in the diagnosis, dynamic observation, evaluation of prognosis and guiding treatment of LC. In this paper, the research on the correlation between conventional CT and serum fibrosis markers was preliminary studyPurpose: To quantitative analys the change of hepatic lobe ratio,portal vein (PV)diameters and splenic volume in patients with LC by Spiral CT,and study the correlation between these CT indexs and serum fibrosis markers.Methods:Totally 50 cases of LC which confirmed by clinical data,laboratory test,type-B ultrasonic or CT examnation liver cirrhosis were divided into 3 groups according to Child-Pugh Classification, namely Child A group, Child B group and Child C group. And there were 25 other patients with normal liver or healthy persons in control group. Spiral CT scan were performed to all study subjects and data was transferred to GE Advantage Workstation(AW)4.1. The hepatic lobe ratio , PV diameters and Splenic volume were observed by processing information with analysis software. In addition, all the LC patients underwent serologic examination, including collagen typeâ…£(â…£-C) and Laminin (LN). The correlations between the ratio of coronal diameters in right and left lobes of the liver (R1/L1), the PV diameter, the splenic volume and the serological indexes were analyzed. Analysis of variance was used in different Child-Pugh grade and control group. The comparison between two groups was performed with LSD test. The pearson correlation analysis test can be used to find the correlations between CT indices of LC and serum fibrosis markers.Results: Compared with control group, the coronal diameter of the left lobe (L1) increased in all LC groups(P<0.01), and there was statistically significant between A and B, C grade(P<0.05), but no between B and C grade(P>0.05). While the coronal diameter R1 and sagittal diameter R2 of the right lobe and Coronal diameter ratio R1/L1 were reduced(P<0.05). The level of R1 , R2 and R1/L1 gradually descent among A, B and C grade and the differences among Child A, B, C grade was significant(P<0.05). The PV diameter and splenic volume in Child A, B and C were significantly higher than those in control group(P<0.01), while there was no statistically different among LC stages(P>0.05). R1/L1 was negatively correlated toâ…£-C and LN in LC patients(r=-0.398, P<0.01; r=-0.568, P<0.01). And there was a positive relationship between portal vein diameter and LN(r=0.490, P<0.01).Conclusions:The coronal and sagittal diameter of right lobe and the Coronal diameter ratio can reflect the clinical severity of cirrhosis. And, the Coronal diameter ratio and PV diameter can indirectly reflect the progress of liver fibrosis.
