Objective:To investigate the expression of high risk human papilloma virus (HPV) 16-E6 protein in normal vulva tissue,Non-neoplastic Epithelial Disorders of Vulva (NNEDV) and squamous cell carcinoma of vulva (VSCC). Explore whether HPV 16-E6 protein is the etiological factor in NNEDV and the correlation of squamous cell carcinoma with vulvae,In order to provide experiment for potential malignant transformation of vulvar.Methods:Using immunohistochemistry SP method to detect HPV 16-E6 protein expression in 15 normal vulvae cases,40 NNEDV cases and 45 VSCC cases.Results:1,The positive rates of HPV16-E6 in different vulva tissue:0% in normal vulva,30% in NNEDV and 66.67% in VSCC, respectively. Use chi-square test to identify the difference of the positive rate between three groups in general (a=0.05;χ2=24.466 P=0.000).The result shows that there has significant difference in general. Use calibration a (a'=0.017) to identify the difference of positive rate between each group. That have.The result shows that there has statistical significance between each group(P<0.017). The Positive material is mainly distributed in nucleus and cell plasma. In NNEDV cases, positive rate of squamous hyperplasia type and lichen sclerosus type are 35% and 25%, respectively. Squamous hyperplasia type and lichen sclerosus type have no statistical significance between them (P>0.05). Which are both significantly higher than that of normal vulvae (P<0.05) and lower than that of VSCC (P<0.05)2,The positive rates of HPV16-E6 in VSCC is 66.67%. The positive rate increased while the clinical stage increasing. Use calibration a (a'=0.017) to identify the difference of positive rate between each stage. Positive rate of stageⅡis significantly higher than that of stageⅠandⅢis significantly higher than that of stageⅠ, respectively (P<0.017) However, stageⅡand stageⅢhave no statistical significance (P>0.017). Positive rate gradually decrease along with the increase of tumor differentiation degree. Positive rate is significantly lower in well differentiated group than that of in poorly differentiated group and moderately differentiated group is lower than that of poorly differentiated group, respectively(P<0.017). However, well differentiates and moderately differentiated have no statistical significance (P>0.017). Positive rate of lymph node metastasis VSCC is significant higher than that of non- lymph node metastasis VSCC, there has statistical significance between each group(P<0.05).Conclusion:1,Positive rate of HPV16-E6 protein in NNEDV are significant higher than that of normal vulvae. As a result, this indicates that the HPV infection may be a etiological factor in NNEDV.2,HPV16-E6 protein has no expression in normal vulva. The positive rates of HPV16-E6 increase gradually in NNEDV and VSCC tissue. The positive rate of HPV16-E6 in VSCC is related to different clinical stages, tumor differentiation degrees and lymph node metastasis. This result infers that the raising of HPV16-E6 positive rate may be related to the occurrence and development of vulvar squamous cell carcinoma.
