Objective: Hypertension disorders complicating pregnancy including five different types of pregnancy:gestational hypertension, preeclampsia, eclampsia, chronic hypertension complicated with preeclampsia and chronic hypertension complicated pregnancy, and preeclampsia is one of the most common type, is the main research object of this article. The diseases affect seriously the maternal and perinatal prognosis, is the main reason which lead to maternal, fetal and neonatal mortality increases. 237 patients with preeclampsia of the Shanxi medical university first hospital were retrospectively analyzed, observe the occurrence of complications and adverse perinatal outcome of different gestational age of patients with preeclampsia, provide guiding for further clinical work.Methods: 237 patients from March 2013 to March 2014 patients with preeclampsia, according to the onset gestational age is divided into four groups: group a(< 28 weeks), group b( gestational age which less than and equal to 28 weeks or less than 32 weeks), group c(gestational age which less than and equal to 32 weeks or less than 34 weeks)(including 2 cases twins),group d(≥34 weeks)(including 12 cases twins). To collect clinical information of study object, including the maternal complications and perinatal outcome and the way of delivery.(because of some newborns were premature babies, and transfer into the childrens’ hospital or pediatrics, telephone follow-up should be paid to the end), by sorting and statistical treatment, were analyzed retrospectively.Results:1. By comparing the clinical complications of 237 patients with preeclampsia, found that the incidence of placental abruption in the four groups was statistically significant(P < 0.05). 2. Through to analysis different gestational age impact on perinatal outcome and found the onset of gestational affect fetal and neonatal outcome, comparing is statistically significant(P < 0.05). 3. To compare the impact which different delivery ways to the fetal and newborn outcome., there was no statistically significant difference(P > 0.05). 4. Statistics the delivery ways of 237 patients with preeclampsia, the proportion of cesarean section is 64.1%, early-onset 33 cases, accounted for 13.9%, 119 cases of late onset accounted for 50.2%; the proportion of vaginal delivery is 35.9%, of which the early-onset 34 cases, accounted for 14.4%, late-onset 51 cases, accounting for 21.5%. To compare the rate of cesarean delivery which gestational age < 34 weeks and ≥34 weeks, between the two groups was statistically significant(c2 = 5.08, P < 0.05), compared vaginal delivery was no statistical significance(P > 0.05).Conclusions: Preeclampsia has serious harm for pregnant women and fetal. The earlier, the illness is more heavier, the more clinical complications, termination of gestation weeks earlier. 34 weeks as an important time for improving perinatal prognosis. Cesarean section is not very effective means for improving the quality of the newborn life, fetal and neonatal complications is rooted in the mother’s illness. The delivery way should be selected on the basis of the specific situation of pregnant women and perinatal decision, and the influence of the cesarean section rate of pregnant weeks. Therefore, strengthening the pre-natal health education, actively participate in perinatal health care is extremely important, can effectively reduce the incidence of serious complications and perinatal mortality; At the same time, try to extend gestational age, timely termination of pregnancy is an important measure to improve maternal and infant outcomes.
