Subject:Monitoring the expression rate of platelet activation markers CD62p,CD63 on arteriosclerosis obliterans patients and the chang during antiplatelet therapy for learn the relationship in patients'platelet activation before and after treatment. The aim of this study will provide rationale fore antiplatelet therapy and make guidance to clinician's drug use based on the expression rate of platelet activation markers.Method:Choose 20 arteriosclerosis obliterans patients during September 2009 to September 2010 whom been treatment in the first affiliated hospital of An Hui medical university, disease stage isⅡ(fontaine) into the experimental group. All the patients accurate diagnosis by color ultrasonic and CTA. Exclude the patient who has the disease will affect coagulation function. The patients were given aspirin enterosoluble tablets 0.1g and clopidogrel hydrochloride tablets 75mg each day orally. By flow cytometry , the expression rate of CD62p and CD63 on the platelet of the patients in the experimental group was tested on the first(before drug used) , third , seventh and fourteenth day for evaluate patient's platelet activation. Choose 20 persons except contract the disease which could affect coagulation function into control group. Check the control group person's CD62p, CD63 expression rate random. Require all the study subjects not used drug which could affect coagulation function. The ankle/brachial index of the patients in the experimental group was calculated on the first and the fourteenth day and distance of intermittent claudication for judgement therapeutic efficacy.Result:1. The expression rate of CD62p in control group is 13.06(±0.43)% , the experimental group is 15.53(±0.80)%. All the results of the analysis used independent sample t test data , the expression rate of CD62p in experimental group is higher than control group(P<0.05).2. The expression rate of CD62p on the platelet of the patients in the experimental group was tested on the first , third , seventh and fourteenth day , positive rate is 15.53(±0.80)%,14.86(±0.68)%,13.94(±0.78)%,13.15(±0.66)%, register as decrease steadily. the data of the analysis used repeated measures analysis of variance, the third day's is lower than frist day's, but the difference have no ignificance(P>0.05). In the the frist day and the seventh day, the frist day and the fourteenth day, the seventh day and the fourteenth, the difference have ignificance(P<0.05).3. The expression rate of CD63 in experimental group is 16.98(±0.91)% , the control group is 15.19(±0.34)%. All the results of the analysis used independent sample t test data , the expression rate of CD63 in experimental group is higher than control group(P<0.05).4. The expression rate of CD63 on the platelet of the patients in the experimental group was tested on the first , third , seventh and fourteenth day , positive rate is 16.98(±0.91)%,16.49(±0.89)%,15.95(±0.99)%,15.24(±0.84)%, manifest as decrease steadily . the data of the analysis used Repeated measures analysis of variance, the third day's is than frist day's, but the difference have no ignificance(P>0.05). In the the frist day and the seventh day, the frist day and the fourteenth day, the seventh day and the fourteenth, the difference have ignificance(P<0.05).5.The ankle/brachial index of the patients in the experimental group was calculated on the first is 0.57(±0.13), the fourteenth day is 0.61(±0.12), the fourteenth day is higher than the frist day. Use independent sample t test data , the difference have ignificance(P<0.05). 6.The distance of intermittent claudication before treatment is 926.5±(833.4)meters, after treatment is 1215.3(±931.7)meters. Contrast two distance, after treatment is longer than before treatment.Use independent sample t test data , the difference have ignificance(P<0.05). This effect could reflect therapeutic efficacy of antiplatelet drugs used in arteriosclerosis obliterans.Conclusion:1. The expression rate of platelet membrane markers CD62p,CD63 in arteriosclerosis obliterans patients is higher than control group,the high expression rate is relevant to arteriosclerosis obliterans. CD62p,CD63 could used for a basis to monitoring the expression rate of platelet activation.2. In occur and developing of arteriosclerosis obliterans, platelet actvation make a important role. The expression rate of membrane markers CD62p,CD63 has falled after use aspirin enterosoluble tablets and clopidogrel hydrochloride tablets, so this two kinds of antiplatelet drugs have the effect of inhibit platelet activation. By constrast the change of patient's ankle/brachial index and the distance of intermittent claudication has increased before and after treatment, we could know antiplatelet therapy have good effect for treat arteriosclerosis obliterans. So the antiplatelet drugs could use for an non-surgical treatment. |
