Comparison Of Ureteroscopic Holmium Laser Lithotripsy And Open Ureterolithotomy For Treatment Of Ureteral Complicated Calculi

Objective:To compare the effect of ureteroscopic holmium:YAG laser lithotripsy and open ureterolithotomy for treatment of ureteral complicated calculi.Methods:From October 2007 to August 2009,clinical data of 62patients(62 sides) with ureteral complicated calculi underwent surgical management were collected and analyzed retrospectively.All patients were divided into two groups respectively according to the operating style,33 patients(33 sides)were treated with ureteroscopic holmium laser and 29 patients(29 sides)underwent open ureterolithotomy.To compare and analyze the operation time,the postoperation recovery time,the hospital stay between the two groups.Results:No serious complications occurred during intraoperation and postoperation in all cases,and long-term complications would be observed.In the group of ureteroscopic holmium:YAG laser lithotripsy,the stone fragmentation rate on one session was 84.85%(28/33).In the group of open ureterolithotomy,the stone free-rate was 100.0%(29/29).The stone fragmentation rate have no statistic difference(P>0.05).But the average operation time,the anodyne after operation,the intestinal functional recovery time,the usage of antibiotics after operation,the urinary catheter extracted time,the hospital stay in the group of ureteroscopic with holmium:YAG laser lithotripsy were better than those in the group of open ureterolithotomy(all P<0.05).Conclusion:Ureteroscopic holmium:YAG laser lithotripsy for ureteral complicated calculi has the advantages of micro-invasive,more safer,short operation time and hospital stay,lessening pain of patient and effective.It is one of the best choices for treating ureteral complicated calculi.Open ureterolithotomy is an important additional procedure for other operating style if the applicability was definite.
