ObjectiveCarcinoma of stomach is the common cancer in the alimentary system. Its etiology and pathogenesis are not known, so it is difficult to carry out primary prevention. It is the key to prevent and treat the disease the secondary prevention from precancerous lesions. It always story longer precancerous lesions phase from normal gastric mucosa to malignant change and the trend is from chronic superficial gastric (CSG), chronic atrophic gastritis (CAG), intestinal metaplasia (IM), dysplasia (DYS) to cancer. The DYS and IM basing on CAG are precancerous. The curative effect of modified Zuojin decoction is marked clinically. The experiment will check the effect and study the possible mechanisms. MethodsRats were classified model group, tretinoin group, high-dose group, middle-dose group and low-dose group that were made the model by drinking MNNG solution freely, being filled with deoxycholic acid natrium, hot and salty water and alcohol, feeding two days and fasting one day for 24 weeks. Then the high-dose group, middle-dose group and low-dose group were treated with the decoction and tretinoin group with tretinoin for 16 weeks. Observed the rats' spirit, appetite, the brightness of hair, stool, weight, death-rate and the change of gastric mucosa in histopathology (including lump, IM, DYS, inflammation, atrophy), detecting the expression of PCNA (proliferation cell nuclear antigen) and bcl-2 protein in gastric mucosa epithelial cell by immunohistochemitry and cell apoptosis by TUNEL, calculated the proliferation index and apoptosis index, compared with the difference among the groups by statistic analysis. ResultsThe model was copied successfully. After being treated, the weight increased markedly, and compared with model group the difference was very significant (P<0.01). The death-rate of model group was up to 54.55% and else group was zero (P<0.01). Lump and IM were not found in high-dose group, low-dose group and tretinoin group, only few light DYS and CAG were found (VS model group P<0.05). In middle-dose group gastric mucosa was generally normal. In tretinoin group inflammation was more serious than in middle-dose group (P<0.05). The expression of PCNA and bcl-2 protein markedly decreased (VS model group P<0.05). Especially middle group can increase AI significantly (VS model group P<0.001) and depress the expressing of PCNA and bcl-2 protein. Conclusion1. Copied the model successfullyChronic atrophic gastritis precancerous is a chronic story. Its occurrence and development relate to many factors, e.g. undue hunger, overeating, salty or pickled food (containing salt, nitrite etc), overheated food and drink, alcohol stimulus and bile regurgitation. In the reports the model was mainly copied by MNNG and together one or two factors above. In the experiment the method of main drug MNNG and therelated factors not only simulate the etiology in human, but also shorten the time, increase the rate of DYS and the model is equable. So it is an ideal method.2. The decoction's good curative effectThe decoction's curative effect was checked by reform the model rats' appetite, increasing the weight, decreasing the death-rate, treating inflammation, atrophy and DYS.3. Possible mechanismCell canceration is characterized by the imbalance of cell proliferation and apoptosis. So it is important to find reliable index to image cell proliferation and apoptosis and to detect PI and AI to study the mechanism. PCNA, a period protein in cell nuclear, is a good index to judge cell proliferation. Bcl-2 is an important gene to regulate cell apoptosis and its innormal expression can depress cell apoptosis. The reports showed the strong expression of PCNA and bcl-2 protein was detected in gastric precancerous lesions. The result showed the decoction can markedly decrease the expression of PCNA and bcl-2 protein and increase AI (VS model group P<0.01). So we speculate PCNA and bcl-2 gene have being activated in precancerous lesions phase and MNNG may result in cell proliferation imlimmitly and cell apoptosis being depressed to aggravate t...
