Diagnosis Of Laryngeal Cancer And Precancerous Lesions And Discussion Of Clinical Treatment Value

Objective: To investigate the value of narrow band imaging endoscopy in the diagnosis of laryngeal cancer and precancerous lesions, and to discuss further the value of clinical treatment value.Methods: Between April 2014 and December 2014, a total of 65 consecutive patients with suspected precancerous or cancerous lesions of the larynx were enrolled in this study. High performance endoscopic system equipped with the white light mode and NBI mode was used in the examination of larynx. All pharyngeal and laryngeal examination was performed under white light endoscopy firstly and then under NBI. Each lesion was observed by NBI endoscopy and taken to the pathological examination. The results were then compared to the corresponding white light and NBI images.Results: A total of 65 patients were detected by white light and NBI modes, compared to the pathological examination. The diagnostic accuracy is 73.30%(10 benign lesions, 14 precancerous lesions, 23 malignance lesions) with white light endoscopy, while the NBI endoscopy is 90.76%(10 benign lesions, 21 precancerous lesions, 28 malignance lesions). NBI was significantly higher than white light endoscopy(x2=7.358,P<0.05). The sensitivity of NBI and white light to detect larynx carcinoma was 90.32% and 74.19%, there was no significant difference between the two groups(x2=2.763,P=0.096). The sensitivity of NBI and white light to detect precancerous lesions was 91.30% and 60.87%, respectively(x2=5.855,P<0.05).Conclusion: The recognition of laryngeal cancer and its precancerous or cancerous lesions and diagnostic accuracy can be improved through NBI technology. NBI endoscopy could not reduce laryngeal cancer and precancerous lesions in the postoperative recurrence rate because of lack of a large number of clinical data. NBI endoscopy may play an important role in clinical treatment.
