Our present research was planed to investigate the effects and mechanisms of Qingluotongbi granule (QLT) on rheumatoid arthritis (RA) according to our previous works.Theory research part was focused on the investigation of the argument, diagnosis and treatment to the syndrome of yin deficiency and stagnation-heat in the collateral channels based on an overall analysis of the illness and the patients condition. We think the RA is characterized by deficiency of liver-yin and kidney-yin, stagnation-heat in the collateral channels and wind-dampness stagnates and stagnation of phlegm and blood stasis. The deficiency of liver-yin and kidney-yin is pathogenesis, while Stagnation-heat in the collateral channels is the superficiality of pathological changes. The main pathological factors are Damp, Phlegm, Blood stasis and Heat. The pathological nature is Deficiency of Origin and Excess of Superficiality. Base on these evidences, we used the pellet named Qing luo tong bi to treat the syndrome of Yin deficiency and stagnation-heat in the collateral channels according to the theory of nourishing the kidney and nourishing Yin, clearing away stagnation-heat and removing the obstructive in the collateral channels and further investigate the mechanism in clinical and animal laboratory.Experimental parts were focused on the effects of QLT on primary and secondary infection, immunol functions, synovitis, apoptosis of the cells of synovium of collagen induced arthritis (CIA) rats and further observe the effect of QLT on the apoptosis of peripheral lymphocyte in the patients with RA. The results showed that the QLT of three different doses could inhibit the primary and secondary infection partly or completely, lighten the histopathologic changes at different degree such as inflammation and cartilage destruction and decrease the level of anti-type II collagen antibody in the sera of CIA rats. QLT 7.2,14.4g/kg treatment could significantly inhibit the increasing of the proliferation of lymphocytes in CIA rats and increase the amount of CD4~+ T lymphocyte while reduced the amount of CD8~+ T lymphocyte and increase the ratio of CD4~+/CD8~+, inhibit the increase of the synoviocytes proliferation in CIA rats. The synoviocyte culture supernatants from QLT 7.2, 14.4g/kg-treated CIA rats could inhibit proliferation of the synovial fibroblast.
