ObjectiveBecause there is a geographical region, climate, culture, and so many different factors between Taiwan and Beijing,This research according to abidance by individuality locality and season ,and is under the base of review literature genealogically, studied with epidemiological method, assessment scale evaluation, to investigate the comparison of symptoms, syndrome and recurrence factors between insomnia patients in Taiwan and Beijing.Methods1. Choosing 200 insomnia patients in both Taiwan and Beijing according to unified diagnostic and exclusion criteria strictly. Comparing ages, gender, education level, professional, symptoms, syndrome and recurrence factors between the insomnia patients in two places.2. Insomnia patients fill in the sleep stage self-rating scale by themselves (SRSS); Special doctors of insomnia do the measuring and fill in the insomnia clinic questionnaire of TCM.Results1. There are 21 male insomnia patients (21%) and 79 female (79%) among the 100 insomnia patients collected by common TCM clinic in Taiwan, the highest occurrence rate is between 31 to 40 years old(23%), second is between 18 to 30 years old(20%), third is between 41 to 50 years old(17%). The patients are mainly middle-young people, their education level are mostly university and even higher educational background, the higher occurrence rate is between mental labor and unemployment people; There are 24 male insomnia patients (24%) and 76 female (76%) among the 100 insomnia patients collected by common TCM clinic in Beijing, the highest occurrence rate is between 41 to 50 years old(23%), second are between 31 to 40 years old, 51 to 60 years old, both are 20%. The patients are mainly middle and old-aged, their education level are mostly high school and higher , the higher occurrence rate is between mental labor and unemployment people.2. The most 10 common symptoms of insomnia patients in Taiwan are: wiry pulse(76%), difficulty falling asleep(54%), dizziness and dim eyesight(52%), restlessness and tantrum(52%), short breath and hypologia (51%), belching(51%), disturbed sleep at night and dreaminess(47%), sigh(45%), depression(45%), hypochondriac pain and gastric upset(43%); The most 10 common symptoms of insomnia patients in Beijing are: wiry pulse(69%), short breath and hypologia (54%), dizziness and dim eyesight(53%), difficulty falling asleep(52%), belching(52%), restlessness and tantrum(50%), disturbed sleep at night and dreaminess(46%), sigh(43%), depression(42%), hypochondriac pain and gastric upset(41%).3. The common syndrome of insomnia patients in Taiwan are: liver-stagnation and spleen-deficiency (33%) , phlegm-heat attacking the heart(25%), obstruction of collaterals by blood stasis(16%); The common syndrome of insomnia patients in Beijing are: liver-stagnation and spleen-deficiency (32%) , obstruction of collaterals by blood stasis(27%), deficiency of both heart and spleen(21%).4. In the SRSS scale, the integral of the third factor(sleep insufficiency or awakening inadequate), the fourth factor(sleep time) and the syndrome of deficiency of both heart and spleen are obviously higher than other 4 syndrome; the patients with the fifth factor (difficulty falling asleep), liver-stagnation and spleen-deficiency or phlegm-heat attacking the heart, their time for falling asleep are more than 2 hours, this proportion is highr than other; the patients who are qi stagnation and blood stasis, their proportion of sleep difficulty are higher than the patients with the sixth factor (degree of easy waken).5. There are 7 contents have relativity with insomnia repeated in its pre-and post self-control table(P>0.05), which are: difficulty falling asleep, easy waken at night or early awakening, poor breath, cough or high snore, nightmare, sleepiness, aneuria. The three items had statistical significance, which are go to the washing room at night or other thing which effect sleep and hypnosis with drug, but the difference is not obvious.Conclusion1. The ratio of male and female insomnia patients is about 1:3, the higher education level the higher incidence, the higher incidence also in mental labor and unemployed people, the patients are mainly middle-young people in Taiwan, but mainly middle and old-aged in Beijing.2. Although the first ten symptoms of insomnia patient in two places have little differences, but basically, liver depression symptoms group is mainly appeared.3. The result of syndrome type comparison of the two places: the common syndrome types of insomnia in Beijing are liver-stagnation and spleen-deficiency, stagnation of brain vessel by blood and deficiency of both heart and spleen; but the common syndrome types of insomnia in Taiwan are liver-stagnation and spleen-deficiency, phlegm-heat attacking the heart and stagnation of brain vessel by blood.4. The syndrome of deficiency of both heart and spleen and qi stagnation and blood stasis are commonly have sleep insufficiency or inadequate awakening; the syndrome of liver-stagnation and spleen-deficiency and phlegm-heat attacking the heart are commonly have difficulty falling asleep; the syndrome of qi stagnation and blood stasis is commonly have easy waken degree.5. These 7 contents may have relativity with insomnia repeated: difficulty falling asleep, easy waken at night or early awakening, poor breath, cough or high snore, nightmare, sleepiness, aneuria.Comparison between Taiwan and Beijing insomnia patients, the difference is mainly manifested in the incidence population, level of education, common Syndrome, the psychological factors; the characteristics of each common Syndrome and recurrence is not significantly different factors.
