Research BackgroudIschemic cerebral vascular disease(ICVD)takes up 80%~85%of cerebral vascular diseases,having high incidence rate and disability rate,seriously affecting healthcare and life quality of middle-aged and aged people.In this recent 30 years,in this field,we have already got much break through and milestone achievements,mainly including there sides:the discovery of risk factors and control methods of ICVD,earlier thrombolysis treatment of cerebral infarction,the use of proctectant and nerve rehabilitation of convalescent.However,many risk factors have been discovered,but still having 30%causes of disease are not sure.Earlier thrombolysis treatment use very limited,and the importance of nerve being assured but not really and regularly applicate in the treatment of cerebral vascular diseases.In addition,with the appearance of aging and hypertension well controlled,the incidence rate of ICVD begins to decrease,but absolute incidence rate and relative rate will step up.For this reason,much work of prevention,diagnosis and treatment of ICVD will be strived to implement.Research ObjectiveTo analyze the distribution of intracranial and extra cranial arteriostenosis in patients with ischemic cerebral vascular disease,explore the relation between risk factors and Intra and extra cranial artery stenosis of ICVD,and provide precaution methods to prevent the disease.Research MethodsTo aim directly at the148 patients of ICVD,these persons had been detected by DSA, to learn about the state of artery stenosis in inside and outside of skull.Taking these cases into three groups,they were respectively as follows:Ⅰgroup,intra and extra cranial artery stenosis;Ⅱgroup,intra cranial artery;andⅢgroup,extra cranial artery stenosis.We measured biochemical indicators such as TG,TC,HDL,LDL,Apo A1,Apo B,CRP,FIB,and so on.,And collected these risk factors with self-design questionnaire such as sex,age,long-term smoking, long-term drinking,hypertension,and son on.Research Results(1)There are 101 cases existing artery stenosis in 148 patients by DSA.Extra cranial artery stenosis has 49 cases(33.1%),intra artery stenosis 23 cases(15.5),intra and extra cranial artery stenosis 29 cases(9.6%).(2)In the 101 patients,single artery stenosis has 35 cases(34.7%),multiple artery stenosis 66 cases(65.3%).(3)In this 101 cases of intra and extra cranial artery stenosis,the most location of artery stenosis as follows:basilar artery and onset part of vertebral artery.(4)The result of DSA detecting indicates the degree of stenosis as follows:there are 198 strips stenosis in 101 cases,light stenosis 84 strips(42A%),midrange stenosis 41 strips(20.7%),severe stenosis 55 stips(27.8%), obliteration 18 strips(9.1%).(5)Extra cranial artery stenosis group:Logistic regression analysis result shows that the risk factors are age(OR=1.124),sex(OR=0.241), hypertension(OR=1.038).Intra cranial artery stenosis group:the risk factor is hyperfibrinogenemia(OR=1.932).Intra and extra artery stenosis group:the risk factors are age(OR=1.084),long-term drinking(OR=6.132),diabetes mellitus(OR=6.106).Research ConclusionIn this study,extra artery stenosis is the most,and multiple artery stenosis is to see frequently.The incidence rate of extra artery stenosis of internal carotid artery is the highest,next is the initial segment artery stenosis of vertebral artery.Different risk factors existing between intra and extra cranial artery stenosis,the risk factors of extra cranial artery stenosis are age,hypertension and male,;the risk factors of intra cranial artery stenosis is hyperfibrinogenemia,and the risk factors of intra and extra cranial artery stenosis are long-term smoking,diabetes mellitus,and age.
